Objective:Ventilator-associated pneumonia(VAP) is the leading intensive care unit(ICU)infection,VAP has increased the length of ICU stay and costs of treatment.To explore the risk factors in patients with ventilator associated pneumonia(VAP) and to provide the corresponding clinical prevention strategies.Methods:The clinical data of all the 78 mechanical ventilation(>48h) patients admitted to the intensive care unit from Jan.2007-Aug.2007 were divided into two groups,with or without VAP,according to the diagnosis criteria.The comparative analysis was performed in risk factors.Using univariate analysis of a statistical software package(PEMS3.1),we first analysed the related risk factors of VAP(including age,coma,mechanical ventilation days,shock, heart failure,COPD,acute reanl failure,diabetes,antibiotic usage,corticosteroid usage, et al),then independent risk factors were determined with backward multivariate stepwise logistic regression analysis.Results:The morbidity of VAP was 46.1%and the mortality was 11.5%,Single factor analysis showed that 9 factors(such as duration of MV,coma,aspiration,et al)were related to the occurrence of VAP.Multi-variable logistic analysis identified aspiration,duration of mechanical ventilation,and combined use of over 2 types of antibiotics as the 3 independent risk factors of VAP.Conclusions: The morbidity and mortality of VAP were very high,VAP was related with many clinical factors.Application of comprehensive management critical surveillancc and combined use of various preventive strategies according to VAP associated factors can prevent and reduce the morbidity of VAP effectively.
