Objective To evaluate the clinical usefulness of graduated 10g monofilament exam,thermal sensibility,vibrayory sensation in the diagnosis of diabetes peripheral neuropathy·Methods The clinical data of 95 patients with type 1 diabetes,type 2 diabetes or impaired glucose tolerance (IGT) (according to the criteria of WHO)and without cerebral vascular,lumbar vertebra disorders and cognitive disorder。10 g monofilament examinations,thermal sensibility,vibrayory sensation,electrophy- siological examination were carried out in all the patients . Using the results of NSV as golden criteria , the sensitivity , specificity , positive and negative predictive values and kappa values of these tests including parallel tests were analyzed . Results The the sensitivity of the seven test(10 g monofilament examinations,thermal sensibility,vibrayory sensation,the parallel test of 10 g monofilament examinations and thermal sensibility,the parallel test of 10 g monofilament examinations and vibrayory sensation,the parallel test of thermal sensibility and vibrayory sensation,the parallel test of 10 g monofilament examinations,thermal sensibility and vibrayory sensation) are 16.2%,89.7%,61.8%,89.7%,63.2%,92.6%,92.6% respectively . specificity were 96.3%,40.7%,85.2%,40.7%,81.5%,33.3%,33.3% respectively. And kappa values were0.077,0.338,0.377,0.338,0.366,0.304,0.304 respectively .Conclusion 10 g monofilament examinations could not be perfect for screening DPN in early stage .Vibrayory sensation is a rapid , simple and reliable test for screening DPN. Uniting thermal sensibility test with 10 g monofilament examinations,vibrayory sensation can make a higher sensitivity.
