Background:Local application of antibiotics has pivotal role in the treatment of purulent otitis media,but its uses and effects in clinics are limited because of potential ototoxicity and drug tolerance.So there is significant clinical value for investigating novel local antibiosis or anti-inflammatory agents, especially contained with activity ingredient of Chinese traditional medicine. Tanshinone,which is a compound of more than 10 kinds of activity ingredient of Chinese traditional medicine,has been proved to have the ability of improving circulation,anti-oxidation,anti-inflammation,et al.Some experiment data reported that tanshinone may reduce ototoxicity of gentamicin on cochlea.But is not clear whether tanshinone has potential ototoxicity and toxic effect on purulent otitis media when it topically use to middle ear cavity.So we extract the active ingredient of tanshinone and make it auristilla,then analysis its effects on mid ear. Firstly,we induced animal model of acute otitis media suppurativa by injection Staphylococcus aureus into the meddle ear,and drop tanshinone auristillae into meddle ear in guinea pig,then study its possible antibiosis and anti-inflammatory effects.Finally,we study whether it has ototoxicity or not.Aims:1.investigate whether or not tanshinone auristillae had any antibiosis and anti-inflammatory effects on acute otitis media suppurativa in guinea pig when it was applicated locally;2.study tanshinone's possible anti-inflammatory effect;3.to verify whether it has ototoxicity.Methods:1.To make acute otitis media suppurativa guinea pigs by injection of Staphylococcus aureus into the middle ear;2.Drop tanshinone auristillae,physiological saline and chloromycin auristillae,respectively into acute suppurativa otitis media of guinea pigs,then determine threshold value of ABR,inflammation excretion score in the meddle ear,bacterial colony account,and observe inflammation cell infiltrate on the frozen section under light microscope and inflammation cell account on the high microscope.3.To evaluate the level of IL-1βby immunohistochemistry method. 4.By control study,to study the frozen section of the meddle ear cavity,scanning electron microscope and stretched preparation on cochlea so as to identify whether or not tanshinone auristillae has any ototoxicity when it is used locally.Results:1. Compared to the physiological saline,threshold value of ABR,inflammation excretion score in the middle ear,and bacterial colony account under high microscope significantly decreased in tanshinone auristillae group,and under microscope.The inflammation cell infiltrate is slighter in tanshinone auristillae group than that in physiological saline group.2.After 3 days later,there is no significant different of threshold value of ABR between tanshinone auristillae and chloromycin auristillae group,but the threshold of ABR has significant different between them after 7 days later;3.In this study,we can find some extent expression of IL-1βin physiological saline,however we have not seen a superfluous expression of IL-1βin tanshinone and chloromycin auristillae group; 4.In the study of ototoxicity,we have found no any evident changes of anatomy in the middle and inner ear in the group of tanshinone auristillae after 7 and 14 days later.Conclusion:1.The result shows that the Chinese traditional medicine tanshinone has a certain effect of anti-inflammation and anti-oxidation in acute suppurative otitis media of guinea pigs in local application;2.The result illuminiates that one of its anti-inflammatory function mechanism may be realized by reducing the expression of IL-β;3.The study shows that tanshinone has no any ototoxicity in the middle and inner ear.
