| PrefaceThe global disease and spectrum of disease has been changed : the decrease burden of acute infectious disease and malnutrition disease and increase burden of chronic disease,senile disease,venereal disease,mental disorder.The greatest proportion of burden is cardiovascular and cerebrovascular disease ,which is known as "high prevalence , high mortality, high mutilation ,high compilation " and is seriously harmful to health and life . This kind of disease has been a serious public health problem. Recently, Ministry of Public Health has surveyed 36 cities and 90 counties ,The data of death monitoring can explain that stroke is main cause of our country . Stroke can cause serious burden to patients,families and society . So it is important to evaluate the burden of stroke accurately . The county of Zhangwu has high prevalence of Hypertention , the standardized rate of hypertention and stroke is 35% and 2606/105.Data and Methods1,The survey of prevalenceCluster sampling was used to identify 11 villages from 6 towns in zhangwu county in Liaoning province.All residents among 18 were surveyed by questionnaires made by unified standard. The sample size was 5208 .The contents of questionnaire included age,sex,history of stroke .If the patient was prevalence , he must provide the evident of diagnose by hospitals ;if the patient was incidence ,he can be diagnosed by unified diagnostic standard made in the fourth academic conference of cerebrovascular disease in 1995.2,The index of the burden of stroke(1)PYLL=sum from i=0 to e[(e-Xi)×di]e: Life expectancy ,Xi: Mid-value of age class , di: Total number of age class(2)WPYLL=∑[(RY-Xi)×di]RY: Retired years, Xi Mid-value of age class, di: Total number of age class(3)VPYLL=∑{[(P0 - P1) - (I0 -I1) - (C0-C1)]}P0: Unproductive years ,P1: Produced years, I0: Non-investment years, I1: Investedyears, C0: Non-consumed years,C1 : Consumed yearsThere are three stages in the life by the global burden of disease ,which includeinvestment: 1-20,production: 21-60 and consumption : 61-。(4)DALYs = YLLs + YLDse : Expectancy of every age class ,D : Disability weight, r: Discount rate, a : Age in which there is disability of disease or death of disease , L : Limit of disability or life lost,β: age parameter, K: ageing weighting, C : Constant.The values of above are unified by the standard of the global burden of disease .The incidence of stroke is rarely measured . To derive the incidence ,we calculate it from one-year prevalence, mortality, case fatality and duration using Dismod software. The incidences of acute stage and non-acute stage of stroke are calculated by the proportion in literature and total incidence. Other parameters such as disability weight,duration and case fatality comes from the estimated of GBD to china.Results 1,The prevalence of stroke was 4397/105; the standardized rate was 2606/105,significantly higher for male than female(3547/105 vs 1902/105) .There is obvious raise after 50 in prevalence .2,The mortality of stroke in Zhangwu county, Liaoning province ,was 224/105,significantly higher for male than female(265/105 vs 183/105). With the increase of age, the mortality rised obviously3,The total PYLL of stroke in Zhangwu county, Liaoning province was 4961.0 person years ,the average PYLL was 5.67 person years ; The total PYLL of male and female was 3186.5 person years and 1774.5 preson years, the average PYLL was 6.22 person years and 4.89 person years respectively. The most PYLL of age classes in male was 60-64(619.5person years) and in female was 50-54(369.0 person years).4,The total WPYLL of stroke in Zhangwu county, Liaoning province was 2082.5 person years , in which the male was 1362.5 person years and the female was 720.0 person years. The most WPYLL was 45-49(350.0 person years )of male .The average WPYLL of 875 strokes in this research was 2.38 person years ,the male and female was 2.66 person years and 1.98 person years .5,The total VPYLL of stroke in Zhangwu county, Liaoning province was -10973 person years ,in which the male and female was -6510 person years and -4463 preson years respectively .The most VPYLL was 35-39(68 person years ) and the least was 60-64(-2200 person years).6,The DALYs per 1000 of stroke in Zhangwu county, Liaoning province was 73.2 significantly higher male than female(41.7 vs31.5).The DALYs per 1000 in male and female in this research was 3.7 and 3.2 times than that of China estimated by GBD2000 , 1.8 and 2.2 times than that of rural area of Liaoning estimated in 2003. The YLDs per 1000 of stroke in this area was 21.3, significantly higher male than female(12.2vs9.1).The most YLDs in male and female of age classes was 60-69(119.3 vs120.8). Conclusion1,In the past 20 years ,the prevalence and mortality of rural residents in Zhangwu county ,Liaoning province, has raised obviously ,higher in male than that of female .There was obvious increase after 50 . With the increase of age ,the prevalence and mortality raised significantly .2,The average PYLL,WPYLL and VPYLL of male in this research was higher than that of female .The most PYLL,WPYLL and VPYLL in male was different from that in female.3,In every age class ,the DALYs of male was higher than that of female .There was obvious increase after 45. The DALYs per 1000 of stroke in Zhangwu county, Liaoning province, was 3.4 times than that of China estimated by GBD2000, was also 2.0 times than that of rural area of Liaoning estimated in 2003.
