A Clinic Study On Using HRT-Ⅱ For The Early Diagnosis Of Primary Open Angle Glaucoma

Objective To study the value of using Heidelberg retinal tomography(HRT-â…¡) for the early diagnosis of primary open angle glaucoma(POAG).Normal eyes,sick eyes suspected of suffering from open angle glaucoma and early primary open angle glaucoma were examined with HRT-â…¡to obtain topography image and some stereometric parameters of optic disc.By comparing these parameters between normal and sick eyes and follow-up to the suspected,the sensitive parameters and its sensitivity of optic disc could be expected to be found.Methods A total of 50 normal eyes from 25 persons,40 eyes from 30 patients suspected of suffering from open angle glaucoma and 48 eyes from 30 patients suffering from early and developing POAG were examined with HRT-â…¡to obtain topography image and some stereometric parameters of optic disc,and their visual field were also observed,then these stereometric parameters of optic disc from normal eyes were compared with that from sick eyes by statistics,the correlation between POAG's stereometric parameters of optic disc and mean deviation(MD) was analyzed.In the follow-up to the suspected open angle glaucoma,changes of the suspected open angle glaucoma were compared and observed.HRT possess variational probability chart alone could observe the small diversification of the suspected open angle glaucoma.Results In comparison with normal eyes,significant difference of cup area,C/Dratio,cup volume,rim area,rim volume,mean cup depth,maximum cup depth,height variation contour,cup shape measure and mean RNFL thickness was found between the patients suffering from POAG and normal eyes.The stereometric parameters of optic disc from SOAG were found between normal eyes and POAG.MD from POAG was consistent with the stereometric parameters of optic disc detected by HRT-â…¡.After 2 years' follow-up,18 eyes from 13 patients were diagnosed as POAG in the patients suffering from suspected open angle glaucoma.In the optic disc's parameters of HRT, rim area,rim volume,height variation contour and mean RNFL thickness were key sensitive target.Conclusions The morphological changes from optic disc of the eyes suffering from POAG can be detected by HRT-â…¡,and correlates with visual field well,so it provides a damaging proof of optic nerve for the early diagnosis of POAG clinically.Combination HRT-â…¡with visual field improves the exact rate of early diagnosis.The development of glaucoma can be found with HRT-â…¡in the follow-up.
