Objective:The purpose of this study is to research an effective method to deal with the secondary unilateral cleft lip nose deformity.Methods:Twenty patients (8 males and 12 females) with ages ranging from 15 to 32, consecutively, with unilateral cleft lip nose deformity between January of 2006 and March of 2008 are included in this study. All of them have received primary correction. At this time we use the general method to correct the secondary unilateral cleft lip nose deformity after deep research of the pathological anatomy. Before operation, routine and local nasal examination should be adopted. Preoperative photographs should be taken. Firstly we mark the new location of the nasal tip and design the incision. Along the V shape incision at the base of the nasal alar, through the skin and subcutaneous skin, care should be taken on adequate release of the lateral crus from its attachments to the pyriform and reattachment to the cheek muscles, reconstruction of the muscle ring across the nasal sill, and reposition of the caudally rotated lateral crus and dome. Across the nasal columella incision we suspend the cleft medial crus and the normal side to reposition the nasal tip,then Use z-plasty to deal with the mucosa goffer in the nostril. The incisions will be closed by 3-0 and 6-0 suture finally. Results:All the patients who received the general method have a good result through the frontal view and the submental view. The surgery also provided significant nasal positioning, and improves nasal symmetry and nostril shape.Conclusions:1,The general method of repairing the secondary unilateral cleft lip nose deformity is effective.2,To base on the pathological anatomy of the secondary unilateral cleft lip nose deformity is very important.3,The operation is simple. There are no severe complication or damage of the nasal cartilage after operation. The technique allows maximum stability in correcting the secondary cleft nasal deformity. The result is good and long lasting, and the scar is not obvious.
