With the improvement of living standards, the incidence of diabetes has increased annually, More diabetic retinopathy (DR) patients are inclined to demand good eyesight. Now laser photocoagulation is the effective means to treat diabetic retinopathy (DR). in DR, by taking different therapeutic methods according to different stages for retinal photocoagulation can we achieve better results.Objective: To investigate the division DR (PPDR, PDR), as well as simple view of diabetic macular edema patients the clinical effect of laser treatment on DR laser photo- coagulation for the treatment time and treatment.Method: According to DRPS (Diabetic Retinopathy Photocoagulation Study Group) to formulate treatment technology, collected by 2003 7-2007, in July, 158 cases of 251 patients with both the Division I fundus fluorescein angiography for the diagnosis and after laser treatment DR cases, except associated with glaucoma, uveal Inflammation: severe cataracts, retinal vein occlusion, and other effects go far in the optic nerve or vision photocoagulation effect of eye disease cases, according to the Third National Eye academic enacted "phased diabetic retinopathy standards" (1) DR phases , photocoagulation after 1, 3, 6 months follow-up of the 1st, after a period of three months follow-up, a follow-up 12-36 months, respect- tively records DR (PPDR, PDR), as well as simple view of diabetic macular edema patients before treatment and laser laser Follow- up after treatment for at least 12 months in the best visual acuity, color fundus and FFA as changes in the inspection, in accordance with standards and FFA vision standards of judgement, and analyzed stati- stically.Results: For patients with PDR PPDR and by visual standards, with laser photocoagulation PPDR postoperative visual acuity improved in 15.1%, unchanged vision for 78.3%; PDR with laser photocoagulation postoperative visual acuity improved in 5.5%, not vision changed to 77.8% in patients with PDR PPDR and overall laser photocoagul- ation postoperative visual acuity improved in 10.3%, unchanged vision for the 78% of patients PPDR effective than laser photocoagulation PDR of the patients, there were significant differences, Uc = 10.4537, P = 0.0012 <0.01 or 0.002. By FFA and color fundus as standard, with laser photocoagulation after PPDR efficiency was 92.5%, with laser photocoagulation after PDR efficiency of 76.9%, PPDR and PDR patients after laser photocoagulation overall efficiency was 84.6%; PPDR period of the effect of laser photocoagulation patients than patients PDR period, there were significant differences, Uc = 9.9362, P = 0.0016 <0.01 or 0.002. The results show that retinal photocoagulation for diabetic retinopathy can be effectively preserve and enhance vision, extensive closed without perfusion, dissipated angiogenesis, the exact effect. During the treatment, we found no complications occured in our patients.It shows it is safe and efficient that we treat patients with DR using panretinal photocoagulation.It is most suitable method for the patients with PPDR .Therefore,the patients with DR should be encouraged to diagnosis and cure as early as possible. For the patients with pure diabetic macular edema, according to the standard of vision, 20% have the improvement of central visual acuity and 66.7% have no change of central visual acuity. According to the standard of FFA and colour picture of fundus, the efficiency of laser photocoagulation is 73.3%.The laser treatment of macula should be done actively in patients with pure diabetic macular edema. Since the structual and fuctional status of macula affect the central visual acuity directly,the time of treatment for the patients with pure diabetic macular edema is very important for improving the central visual acuity and preserving fuction of macula.Conclution:1.Retinal photocoagulation is the most safe and effictive treatment for DR.2.The key of cure the DR with laser is to recognize the optimal therapy time which is in the PPDR stage. Diabetic retinopathy should be diagnosed early and treated properly by photocoagulation in time.3.According to the different stages and severity of this disease, we can treat it with different method of photocoagul - ation. The sub- panretinal photocoagulation or panretinal photocoagulation is a better referral than others. In PDR stage,it is better to use panretianl coagulation. Additionally, the patient with pure diabetic macular edema can be treated by focal photocoagulation or grid pattern argon laser macular coagulation.4. Laser treatment should be initiated early and actively in the patient with pure diabetic macular .It is effective to save and improve the vision acuity.5.The patient with DR must be followed-up so that the added photocoagulation can be done with the change of this disease.
