In recent year, people pay close attention to gyneic procreate health along with the improvement of the iatric and living standard , childbirth without pain (CWP) become the thing that the puerpera and the medical staff to follow with interests ,laughing gas CWP is a kind of means that is safe,convenient and effective. Laughing gas is nitrous oxide(N2O),it is a kind of inhalant paregoric or anesthetic with minimal toxicity . For a long-term clinical research , N2O can relieve labor pain and to shorten birth process remarkably . The pharmacological action principle of N2O is inhibit releasing of central nervous system (C.N.S) excitability neurotransmitter and the conduction of nerve impulse and changing ion channel permeability . It can lessen tiredness,anxiety and tension caused by labor pain,It also can lessen the puerpera,s stress reaction and energy consumption .But the problem we were to be faced with is :If it has any effect to the milk secretion about N2O CWP·Breast milk is the most ideal natural eatables to the infant , it contains sufficient nutritional ingredients and it can easy to be digested and absorbed ,it can inhibit bacteria , it enhances absorption of calcium content , it increases disease resistance , and it is benefits to cerebral neural development,intelligence and physical development, at the same time breast feeding is benefit to mother health . lacteal formation and excrete is a neuroendocrine adjusting process effected by internal secretion,shengli,environment and heredity etc . The concentration of prolactin in blood serum plays a decisive role to milk secretion , Human serum pituitary appendage prolactin is proteohormone that secreted by anterior pituitarium , it plays an important role in starting the milk secretion with the help of estrogen,progestin,corticosteroid and insulin . prolactin in human serum increased gradually during the gestation , it reaches the peak prior to delievery , it increased further during lactation .From this clinical observation we can conclude : CWP has no harmful effect to concentration of blood serum prolactin , it can relieve the ischesis of prolactin , prolactin secretion increases more than before ante partum after post partum 24 hours , puerpera is satisfacted with the childbirth process because using CWP , so they can go with medical staff actively after parturition , Mother and her baby are easy to do premature contact and to suck milk early , it is helpful to colostrum feeding and neonatus development .Objective: to approach the changing of prolactin level in blood serum and the changing of puerperal lactation after appraising the application of N2O CWP in labor process , it provid some certain clinical evidence for if we can do the breast feeding successfully . Mehtod: to devide 58 vaginal delivery primipara into two groups , one is N2O group (N2OG) the other is control group (CG), the primipara in N2OG inhalat N2O during labor process , the primipara in CG did not use any anesthetic agent,paregoric . We take vein blood 5ml from the primipara when antepartum (when regularly uterine contraction begins ),post partum 5 minutes and post partum 24 hours , to detect prolactin level in blood serum by radioimmunity , to compare prolactin level of two groups , to compare the Apgar of neonate in two groups , to observe the milk secretion,breast-feeding condition,neonate evacuation and physiological weight lose in two groups at post partum 24 hours,48 hours and 72 hours . Result: It has no significant difference between N2OG and CG about prolactin level ( P>0.05 ) . It has significant difference between prolactin level of N2OG and prolactin level of CG after post partum 24 hours ( P<0.05 ) . It has no significant difference between neonate Apgar of N2OG and neonate Apgar of CG ( P>0.05 ) . It has no significant difference between the begining of milk secretion of N2OG and the begining of milk secretion of CG , but the milk secretion of N2OG is more than the milk secretion of CG . The physiological weight lose of N2OG is lower than the physiological weight lose of CG .Conclusion: N2O can promote the breast milk secretion after parturition , It has some clinical reference value to help we perform the breast feeding successfully .
