| Chronic sinusitis and polyps are kinds of diseases that can threaten people's health and lives seriously.They are also common,frequently encountered diseases with an incidence rate of 8%～15%.Furthmore,the number is growing year by year. Chronic sinusitis and polyps are not only correlated with many kinds of diseases which can harm human's health,but also can bring about chronic hypoxia with the reason of narrow or obstruct nasal passage.This will influence patients'livelihood,work and learning.That is to say chronic sinusitis and polyps will influence patients'quality of life extremely.So it is important to evaluate its therapeutic result.With the development and application of FESS,it is agreed on the therapeutic effect of Chronic sinusitis and polyps.Moden medical model has transformed from biomedicine to bio-psycho-social medicine.People start to know healthy concept.That is we not only evaluate organ function with biological parameter,but also use psychological and sociological parameter in order to obtain full-scale appraisal documents.Quality of life,as a composite parameter,turns out with a background of improvement of objective health level and renewal of health concept.Moreover,people pay more and more attentions to it.Up to now,studies of disease relatedto quality of life have used in therapeutic effect and regimen evaluation of clinical test about cancer and chronic diseases.Recent years,there are more and more documents abroad about Chronic sinusitis and polyps'influence to human's quality of life.So we can take the concept of quality of life into the study of prevention and cure about the disease.Then we can consummate its therapeutic effect evaluation.My study is from patients'angle,use the method of the study of quality of life and MOS SF-36 and modified SNOT-20 to study the patinets'variation of QOL pre&post-operation in Northeast.Then we can evaluate the therapeutic effect of FESS and provide information for clinical therapy of patients in Northeast.Objective To implore the positive effects of FESS on the quality of life(QOL) of patients with chronic sinusitis and polyps,and direct clinical therapy.Methods A randomized and controlled trial was conducted to survey QOL ststus of fifty-two chronic sinusitis and polyps patients undergoing FESS in the E.N.T department of the second hospital of Jilin University,at the baseline and at three months postoperatively by QOL shortform 36-item health survey(SF-36) and sino-nasal outcome test-20(SNOT-20).Results After FESS therapy in the three minths follow-up settings,general health of SF-36 and total of the 20 items scores all get better markedly.In SF-36,the dimensions of PF,SF,VT,RE,GH and total of the scores are improved compared with that of preoperation(P<0.05).In SNOT-20,nasal obstruction,lack of a good night's sleep and etc. and total scores are decreased markedly compared with that of preoperation.Dizziness,sneezing and etc. are decreased compared with that of preoperation.At the baseline ,the most important 5-item is nasal obstruction,need to blow nose,thick nasal discharge,dysosphresia,dizziness.At three months postoperatively,the most important 5-item and its sequence becomes thick nasal discharge, dysosphresia, need to blow nose,post-nasal discharge, nasal obstruction.Conclusions Patients with chronic sinusitis and polyps in northeast get better markedly compared with that of preoperation.FESS is an effective therapy for chronic sinusitis and polyp.Nasal obstruction has improved markedly,but thick nasal discharge,dysosphresia and need to blow nose etc. have not solved effectively yet.That is to say,although FESS can improve patients'QOL,it cannot solve the problem thoroughly.So we need a combined therapy to improve patients'QOL. |
