Clinical Application Of Mini-incision During Unroofing And Decompression Of Renal Cyst

Kidney is the most vulnerable to the human body organs of the cyst,the simple renal cysts is the most common cystic kidney disease, higher incidence, according to statistics foreign research institutions people over 40 years incidence rate is 20 percent, people over 60 years incidence rate is 33 percent;with the economic level ,health conditions ,the impact of modern science and technology development,the detection rate and the diagnosis rate of renal cysts significantly improve. For larger cyst , clinical symptoms , renal parenchyma oppressed broader and heavier,complications occurred,suspected malignant or patients that medical examination found can not afford psychological press requirements treatment , require Surgical treatment. Treatments include the percutaneous puncture and hardening of renal cyst,the traditional unroofing and decompression of renal cyst through waist,the laparoscopic unroofing and decompression of renal cyst and the mini-incision during unroofing and decompression of renal cyst and so on;because the percutaneous puncture and hardening of renal cyst has higher recurrence rate,so its usage gradually less in clinical application.In the clinic working we apply the mini-incision during unroofing and decompression of renal cyst,we retrospectively analyzed the cases of simple renal cysts from in January 2006 to in December 2007,compared with the traditional operation through waist(24 cases) and the retroperitonesal laparoscopic operation(50 cases),we applied the mini-incision operation to treat simple renal cysts(32 cases);through in operative time,blood loss, hospital stay, postoperative recovery, intraoperative and postoperative complications,the cure rate and recurrence,the mini-incision operation respectively compared with traditional unroofing and decompression of renal cyst and the laparoscopic unroofing and decompression of renal cyst, discussing clinic application of mimi-incision during unroofing and decompression of renal cyst. Traditional operation group,retroperitonesal laparoscopic operation group and mini-incision operation group the average operative time was respectively(80.5±22.6)min,(50.7±17.5)min and(45.5±16.7)min;average volume of blood loss was respectively(124.0±30.7)ml,(40.2±13.8)ml and(50.7±15.2)ml;the average postoperative hospital stay was respectively(8.0±1.1)d,(4.8±0.7)d and(4.5±0.6)d;after statisticallyin postoperative recovery the traditional operation was slow , but the retroperitonesal laparoscopic operation and the mini-incision operation were fast;in postoperative symptoms improving, intraoperative and postoperative complications,the cure rate and recurrence,three groups had not significant difference.Based on the above results we think the traditional unroofing and decompression of renal cyst through waist apply patients that have surgical indications,disadvantages include the surgery a long time, trauma, recovered slowly, a longer duration of hospitalization, patients are not accepted. The retroperitonesal laparoscopic operation is smaller scope in surgical indications than the former, benefits include less trauma, less blood loss, quicker recovery, shorter hospital stay and reduce the general open surgery complications, surgical results is identical with open surgery, patients and clinicians easily accept;disadvantages include expensive equipment,operators require a higher level of technology ,require under general anesthesia, with the financial burden of weight, occur subcutaneous emphysema, retroperitoneal hematoma, damage the surrounding organs, air embolism and high CO2 blood disease, and hernia , such as deep vein thrombosis, operative and postoperative complications, it is difficult to the grass-roots level hospital.The mini-incision during unroofing and decompression of renal cyst is too smaller scope in surgical indications than the traditional operation, the best indication is kidney pole cyst and central part renal cyst; benefits include shorter operative time, less trauma, quicker recovery, shorter hospital stay,less postoperative complications, the operation is simple, easy to grasp, without complicated equipment, effective, it is easy to the grass-roots level hospital.To sum up, the mini-incision operation integrate the advantages traditional open surgery and laparoscopic surgery;it is simple, minimally invasive, rapid postoperative recovery, clinical sure,do not require the establishment of pneumoperitoneum and no pneumoperitoneum complication,with the possibility and worth promoting in clinical work.In fact, mini-incision surgery is improvement of traditional surgery,it is a minimally invasive treatment method.
