With rapid development of microsurgical instruments and phacoemulsification , the Progress of materials and technology progress intraocular Phacoemulsification combined with intraocular lens implantation has not been simply consdered as a treatment of blindness, but as a refractive surgery by a growing number of scholars.with the convenient and safe that it brings, Ophthalmologists and patients themselves are gradually beginning to pay attention to postsurgerical visual quality, for that Wavefront aberration become an important evaluation index. In this study, we compare the best corrected visual acuity(BCVA),higher-oerder wavefronet aberrations and the operative complications of patients,to obtain an objective evaluation of the postsurgerical visual quality of the axial high myopia cataract patients.Phacoemulsification with intraocular lens implantation was performed on 83 eyes of 62 patients with cataract from May 2006 to October 2007. It was divided into two groups according to ocular axial length,Group A the ocular axial length 22mm-24mm, Group B the ocular axial≥26mm.Postoperative the best crected visual acuity(BCVA), higher-oerder wavefronet aberrations and the operative complications of patients were observed and then compare the differences between Group A and Group B.Examinations of the BCVA and higher-order wavefront aberrations were carried out between 3 month and 10 month after the surgery.The BCVA of Group A was 0.7±0.12, The BCVA of Group B was 0.5±1.58. The BCVA of Group A was better than Group B. For a 5.0mm pupil, the Spherical Aberration, RMS4, RMS5, RMS6 and RMSh of Group A and Group showed significant Statistical differences,however there were no significant Statistical differences in the examination of Coma, Trefoil and RMS3. Both Group A and Group B were satisfied with the postoperative visual acuity. Especially Group B, some of them considered their visual acuities exceeded their expect.The clinical results confirm the significant reduction of the Spherical Aberration, RMS4, RMS5, RMS6 and RMSh in eyes implanted with Group A than Group B, especially the Spherical Aberration we also conclud that Group A had less rate of the postsurgrical halo than Group B.
