Objective:Shock caused by drug sensitivity(SCDS) is the most frequent reason for drug caused sudden death, has high frequency and very dangerous, it always cause bother of medical treatment, and is frequent in clinical and medical jurisprudence. So it is the main part in medical jurisprudence for SCDS.Tryptase is proteinase presynthesis in mast cell, release granules from mast cell, participate the inflammation expecially the allergic inflammation with other media. These media in one way can cause clinical reaction of allergy, in the other way, it is the evidence of activation the mast cell, can help the diagnosis of allergic disease.There is less report on the immunohistologic research about tryptase and TNF-αon SCDS in recently, in this research , we set up the guinea pig model of SCDS by BPO protein, imitate the pathologic progress of the human pathophysiologic progress of SCDS, separate the animal group according to <>,stain the lung tracheas stomach tissue by immunohistochemistry, observe the expression of tryptase TNF-αof corpse reserve by different method, study the IgE changes in serum by microlocalization and qualitative analysis, discuss the tryptase and TNF-αexpress change and IgE level in serum of corpse caused by SCDS, in order to provide the research basis for medical jurisprudence.Methods:Select 30 health guinea pigs, weight 200-300g, after feeding for one week, separate for 2 groups randomly, 15 for test group, 15 for control group and then separate for 3 subgroups according to dead time, immediately after death, refrigerate 48 hours under 4℃, frozen for 7 days, each group include 5 guinea pigs.Prepare the BPO-BSA and BPO, set up the animal model by abdominal injecting 0.2% BPO-BSA(1:10)0.5ml, tertian for 3 times, saline for control group .At last time, inject 0.2%BPO-HAS 1ml in tail vein, after two weeks make animal death for all allergic shock, take lung tracheas and stomach tissue for fixation and paraffine slice, after death immediately, refrigerate 48 hours under 4℃,frozen for 7 days, centrifuge the serum immediately after death.Test the expression of TNF-αand tryptse in lung tracheas and stomach tissue, observe the IgE changes in serum by ELISA; analyze the distribution and content of tryptase and TNF-αin lung tracheas and stomach tissue, IgE changes in serum, to provide the theory basis and morphologic guideline for medical jurisprudence.Results:The expression of tryptase and TNF-αboost up in lung tracheas and stomach tissue of guinea pigs of SCDS, dispersion significantly to structureSpace. There is no difference in test result between refrigerate 48 hours under 4℃or frozen for 7 days. IgE level increase signif- icant in serum for SCDS group.DiscussionSCDS is severe allergic reaction of typeΙ, allergic sudden death caused by SCDS is very popular in clinical and medical jurisprudence. It has been a difficulty to find out a reliable and accurate diagnostic method and guideline for SCDS.Tryptase release is a reliable marker for mast cell activation, it's also a useful guideline for evaluate allergic disease. Although the difference between environment and reserve method, the degradation of tryptase is not significant, so the value of diagnosis of allergic reaction is certificated. In this research, we find out the area of stain in lung tracheas and stomach tissue is increased immediately after death caused by SCDS; the mast cell increase, Tryptase stain shallow, disperse to structure space. It's similar between the expression of Tryptase in group of refrigerate 48 hours under 4℃or frozen for 7 days. TNF-αis a kind of inflammation factor produced by monocyte and macrophage. proper TNF-αcan protect the body, but large amount of TNF-αor chronic stimulation will cause the immuno-pathologic injury of the tissue. There is nearly related between TNF-αand allergic disease, for many immune cells and inflammation factors participate the disease of allergic shock. In this research we find out that positive production of immu- nohistochemistry increase significantly in lung tracheas tissue of SCDS group, the expression is similar between refrigerate 48 hours under 4℃or frozen for 7 days.No expression in stomach tissueIgE is a very important media for typeΙallergic reaction, it also called reagent or cellphilic antibody, is a kind of secretion globin, if IgE increase in serum, it always presence the allergic heredity. So IgE increase in serum is a assistant diagnostic guideline for diagnosis of SCDS.Conclusion:Tryptase increased significantly in lung tracheas and stomach tissue of guinea pig group of SCDS, TNF-αincreased significantly in lung tracheas, disperse to structure space, there is no influence between refrigerate and frozen condition, so the immuno- histoche- mistry stain can make for morphologic guideline of SCDS. IgE increase in serum can be a assistant diagnostic guideline.
