Objective: To discuss plasma rennin activity and plasma cortisol density through general anesthesia and general anesthesia combined with epidural anesthesia for hypertensive patients undergoing gynecology laparoscopy.Methods: We randomly assigned 30 ASA I-II patients of essential hypertension, aged 30-55yr and weighed 52-76kg who underwent gynecological laparoscopy to receive either general anesthesia with remifentanil target controlled infusion (TCI) (group G) or general anesthesia with remifentanil TCI combined with epidural anesthesia (group GE), with each 15 patients. Blood pressure as well as the concentration of blood plasma (PRA), angiotenson (ATII), aldosterone (Al) and cortisol (COS) were monitored pre-anesthesia(T1), the moment after intubation (T2), 20 minutes after aeroperitoneum with CO2 (T3), 10 minutes after relief of aeroperitoneum with CO2 ( the moment operation finished ) (T4).Results: There were no differences of the patients condition pre-operation. Systemic blood pressure (SBP) and diastolic blood pressure ( DBP ) at T3 and T4 of group G were both significantly higher than that in group GE ( p<0.05 ). As to the group G, SBP at T3 and T4 were higher than that at T1 which showed statistically differences (po.o5). In group G, HR at T3 was higher than that at T1 which was significantly different (p<0.05). The concentration of PRA, ATII, AL and Cos at T3 and T4 in group G were higher than that in group GE (p<0.05). In group G, the concentration of PRA, ATII, AL and Cos at T3 and T4 were higher than that at T1 which presented to be significantly different (p<0.05).Conclusion: General anesthesia combined with epidural anesthesia could effectively reduce the release of PRA, ATII, AL and COS, which inhibited stress responses in gynecology laparoscopy caused by aeroperitoneum with CO2, and was benefit in homoeostasis and protection of organ functions.
