Surgical Treatment And Recurrence Of Giant Cell Tumor Of Bone

Background:Giant cell tumor of bone (GCT) is frequently seen in bone tumors, with local aggression and high risk of recurrence. The factors of recurrence includes imaging grading, biological behavior, modus operandi,adjunctive therapy, and etc. But the radical cause is the residue of tumor cells. The essentiality of clinic study on recurrence is to evaluate in total, and determine the reasonable operating style and step down the recurrence rate.Objective: Retrospectively analyze on cases of GCT, get an impression on the factors of recurrence .Evaluate the effect of modus operandi and the characters of the cases with recurrence. Accordingly provide evidences of surgical treatment on GCT.Methods: Review on 63 cases of GCT with follow-up which were selected from 80 random cases from 1985.1-2003.12. Intra-lesion curettage was applied on 37 Enneckingâ… -â…¡cases, 7 cases recurred. Another 11 Enneckingâ… -â…¡cases with were applied by wide en bloc resection ,without recurrence. Intra-lesion curettage was applied on 4 Enneckingâ…¢cases, 2 cases recured. Another 8 Enneckingâ…¢cases of were applied by wide en bloc resection ,1 case recured.Results: In the same clinical surgery staging, there are no significant deviation between the group with curettage and the group with wide en bloc resection; After curettage, while the adjuvant technique was applied, cavity packed by bone graft or bone cement, that was no deference. According to analysis on cases of recurrence, in the cases which recurred once again, the clinical surgery staging set up, while the style of operation was more aggressive.Conclusion: Reccurrence of GCT intimately correlates with its biological behavior and surgical treatment. Clinical surgery staging is still significant in directions of therapy. Extended curettage,adjuvant and modern imaging techniques improved local recurrence rates .Curettage is still the preferred treatment for most cases of GCT.
