Objective:To investigate the diagnostic value of DNA analysis with flow cytometry in malignant pleural effusions and evaluate the diagnostic value and clinical feasibility of DNA analysis combined the CEA and SF detection in malignant pleural effusions.Methods: The benign and malignant pleural effusions from 78 patient were detected by DNA analysis using flow cytometry and were examined for CEA and SF level by CLIA at the same time.Result:The sensitivity of the DNA heteroploid detection with flow cytometry was 84.21% , the specificity was 80% ,the Youden's index was 0.64 , the accuracy was 82.05% and the positive predictive value was 80%.The sensitivity of the CEA detection was 60.53% , the specificity was 97.5%, the Youden's index was 0.58 ,the accuracy was 79.49% and the positive predictive value was 95.83% . The sensitivity of the SF detection was 94.74% , the specificity was 50%, the Youden's index was 0.45 , the accuracy was 71.79% and the positive predictive value was 64.29%.There is significant difference in the sensitivity, specificity and positive predictive value among the three detections(P<0.05),but there is not significant difference in the Youden's index and accuracy(P>0.05). The DNA analysis connects the CEA detection in parallel test(Its sensitivity was 93.77%,the specificity was 78% ,the Youden's index was 0.72 , the accuracy was 85.68% and the positive predictive value was 80.19%.) and contacts the SF methods in series test(Its sensitivity was 79.78%,the specificity was 90% ,the Youden's index was 0.70 , the accuracy was 85.02% and the positive predictive value was 88.34%.) have better diagnostic value for malignant pleural effusions among the all combine test.Conclusions:DNA anlysis using Flow Cytometry to the malignant pleural effusions is highly sensitive and specific ,therefore ,it is good value detection for malignant pleural effusions and can make up the shortcoming of cytology.The CEA or SF detection is limited for diagnosis of malignant pleural effusions; It is better helpful to diagnosis of the malignant pleural effusions when DNA anlysis combined the CEA or SF detection.
