【0bjective】To investigate the change law of the serum level of heart fatty acid bin- ding protein (HFABP) during the perioperation of open-heart surgery with cardiopul- monary bypass (CPB )and the clinical value of HFABP in the early detection of myo- cardial injury following open-heart surgery with CPB.【Methods】Eighteen patients following date-selectived open-heart surgery by CPB were enrolled at admission for the test group. Blood samples were collected at 5 time points from prior to CPB to 24h after aortic declamping (prior to CPB…T1,30min…T2,3h…T3,12h…T4 and 24h…T5 after aortic declamping) and determined serum levels of HFABP,creatine kinade isoenzyme-myocardial band (CK-MB) and cardiac troponin I (cTnI).The serum levels of the three markers were also determined in 20 healthy subjects selectived randomly in our hospital as the control group. The change of the serum levels of these three markers at different time points was analyzed and compared between the study group and the control group,so as to explore respective- ly the correlation between the peak level of HFABP and the peak level of CK-MB, the peak level of cTnI,ACCT and the dose of Dopamine used after operation.【Results】Compared the serum level of all the markers in the experiment group with that in the control group, the serum concentration of HFABP in the experiment group elevated obviously at T3,T4 and reached its peak at T3,and then decreased nearly to the normal level at T5;the level of CK-MB in the experiment group increa- sed remarkabely at T4, T5 and reached its peak at T4 and was still higher than that in the control group at T5;as far as the level of cTnI in the study group was conce- rned ,it ascended clearly at T4 and reached to the peak at T5.There was significant dif ference of peak time point among these three markers:HFABP peaked much earlier than CK-MB and cTnI. The peak level of HFABP has a positve relationship with that of CK-MB,cTnI, and ACCT,but not with the dose of Dopamine used after oper- ation.【Conclusion】HFABP,a newly-known biomarker for the detection of myocardial injury following ischaemic,is one of suitable biomarkers and superior to CK-MB and cTnI in the early detection of myocardial injury following open-heart surgery with CPB,which means it is of practical value in diagnosing the perioperative myocardial injury severity,prognosing the operation and assessing the effect of the myocardial protection.
