Statistics On The Hospitalizations For Diabetes As Any-listed Diagnosis In West China Hospital, 1996-2005

Objectives: To estimate the number of hospital discharges with diabetes as any-listed diagnosis in all patients hospitalized in West China Hospital (WCH), 1996-2005, and analyze the causes of hospitalizations, the frequency of diabetic complications and the accompanied diseases,and the risk factors of chronic diabetic complications. We hope the study could provide any valuable information about diabetes epidemic trend for our hosipital.Objects and Methods: Collected and analyzed half of the medical records of diabetic patients, who were admitted during 1996~2005 in WCH.Results and Conclusion: The proportion of hospitalizations with diabetes in all patients hospitalized in1996 was 3.3%,and it was up to 5.4% in 2005.The total number of hospitalizations with diabetes increased from 845 in 1996 to 4027 in 2005,which increased 3.8 folds during this period. The proportion of in-pations aged 60 years and older with diabetes in all diabetic patients in 1996 was 58.6%, and it was up to 60.0% in 2005.The first cause of hospitalization in diabetic patients was chronic complications ( 20.2%),and the infecton (19.5%) ,hyperglycemic symptoms were followed.However,the proportion of acute diabetic complications was only 5.8%.The information of chronic compications in hospitalizations with diabetes was as follows: more than 50% of diabetic patients didn't examine the eyeground,and the value of patients with diabetes who did't screen the microalbuminuria was beyond 80%.Among the diabetic patients who had examined the eyeground or screened the microalbuminuria, the ratios of diabetic retinopathy and microalbuminuria were 57.9%, 49.0% respectively. The prevalences of diabetic nephropathy, diabetic neuropathy and diabetic macrovascular diseases were 12.2%, 16.5%, 22.9% respectively. The prevalence of infection was between 34.6%~44.0% in recent 10 years. There were more than 70% of hospitalizations with diabetes had dyslipidemia. conclusion: The proportion of diabetic patients in all hospitalizations increased,the same trend of in-patients with diabetes aged 60 and older was found.The main causes of hospitalization in diabetic patients were chronic complications,infection and hyperglycemic symptoms in recent 10 years. With the decrease of acute diabetic complications, the prevalence of chronic diabetic complications increased gradually in hospitalizations with diabetes.The ratios of patients who did't examine the eyegroud or screen the microalbuminuria in all diabetic pations were high. Fortunately, the ratio of unexamined the eyegroude had decreased obviously, comparing with the consistently high ratio of microalbuminuria unscreened. The prevalence of infection was consistently high during this period. Meanwhile, the prevalences of hypertension,dyslipidemia and malignant tumor also rose rapidly. The common risk factors associated with chronic diabetic complications were age, duration of diabetes and high systolic blood pressure.
