Objective: Implement the health management for diabetes patients, compare the different of the objective indexs, medical cost ,and knowledge, behavior in the intervention group after the intervention, compare the different between the intervention group and control group after intervention. Estimate the effect of the health management from comprehensive aspects. Explore the effective management pattern for the patients with diabetes mellitus.Method: 114 diabetes mellitus outpatients were investigated with questionnaire, personal examination. 58 were intervention group and the other 56 were control group. Intervention group were intervened for 3 months including lectures twice every week, informal discussion, consultation via telephone, mental help, compile the personal health file, and make up the personal intervention scheme respectively et. The examination indexs were length, weight, twist to hip ratio, blood pressure, fasting plasma glucose, postchallenge plasma glucose, Hba1c et. Use the Excel 2000 to register the data and the SAS8.0 to analysis the data.Result: After 3 months health management intervention for the intervention group, the BMI, the blood pressure, Hba1c, FPG, postchallenge plasma glucose of intervention group were decreased, and the change of Hba1c, FPG, postchallenge plasma glucose were significant(P<0.05), while the former indexes of control group were increased, the change of low blood pressure were significant(P<0.05). After 3 months intervention, the medical cost of intervention group were decreased and the difference were significant, while the medical cost of the control group were increased and the difference were significant(P<0.05). After 3 months intervention, the knowledge and behavior of intervention group were increased and the change were significant(P<0.05), while the knowledge of control group were increased and the behavior of control group were decreased, the change of knowledge were significant(P<0.05), but the difference of change of knowledge and behavior between the two group were significant(P<0.05). The main factors to knowledge were education, disease time, behavior. After 3 months intervention, the living quality and anxiety of intervention group were decreased and the change were significant(P<0.05), while the living quality and anxiety of control group were increased.Conclusion: The indexes of control group were worse after 3 months(although they hospitalize and dose). After the normal health management, the indexes of intervention group were not worst and were better. It is proved that the diabetes health managemnt were very important and effective. The model of health management were worth studying and popularizing.
