Objective:To analyzed the expression of leptin in gastic mucosa of H.pylori infected gastritis and gastric ulcer,to explore the correlation of leptin and Helicobacter pylori.And to study the difference of leptin of gastric mucosa in adult and children after H.pylori infection.Methods:(1)87 adlut and 59 children with upper abdominal discomfort were recruited in this study,endoscopy was carried out on all subjects.Total number of 87 adult was included and divided at random into two subgroups,49 chronic gastritis patiens and 38 gastric ulcer patiens.We also studied 59children with chronic gastritis.Immunocytochemistry was used to detect the expression of leptin in the mucosal biopsies from the corpus ventriculi and gastric antrum of patients with H.pylori infected and uninfected chronic gastritis,and the leptin expression was deteced in gastric ulcer surrounding mucosa.(2)Helicobacter pylori infection was diagnosed use rapid urease-nessler's test and Warthin-Starry sliver staining.Result:(1)49 Chronic gastritis patiens included 25 H.pylori positive patiens and 24 uninfected ones.38 Gastric ulcer included 22 H.pylori positive patiens and 16 uninfected ones.59 children with chronic gastritis,was divided into two subgroups,31 H.pylori infected and 28 uninfected ones.(2)The dectection rate of H.pylori in children chronic gastritis reached 52.50%.(3)The expression of leptin was significantly increased in the corpus ventriculi mucosa of adult H.pylori infection patients including adult and children with chronic gastritis as compared with that in the uninfected ones(x~2=5.529 P=0.019, x~2=10.774 P=0.001).The expression of leptin was no significantly difference in the antral mucosa of adult H.pylori infection patients including adult and children with chronic gastritis as compared with that in the uninfected ones (x~2=0.589 P=0.444,x~2=1.883 P=0.170).The expression of leptin had no significant difference in the gastric ulcer surrounding mucosa between in H.pylori infected and uninfected ones(P=0.632).The expression of leptin had no significant difference in the gastric antral and mucosacorpus ventricul between in adlut and chidren H.pylori infection patients with chronic gastritis(x~2=0.024 P=0.877,x~2=0.064 P=0.801).The expression of leptin had no significant difference in the gastric antral and corpus ventriculi mucosa between in adlut and children H.pylori uninfected chronic chronic gastritis(x~2=0.437 P=0.508, x~2=0.008 P=0.931).Conclusion:The dectection rate of H.pylori in children chronic gastritis reached 52.50%.Leptin is present in the human gastric.H.pylori-associated gastritis leads to significant increases in local leptin concentration in the corpus ventriculi.H.pylori-associated gastric ulcer leads to no difference in the gastric ulcer surrounding mucosa.These provide further evidence for the involvement of gastric leptin the contribution to the occurrence and development of H.pylori-associated gastritis.Chronic gastris leads to no significant difference in local leptin concentration between adult and children gastric mucosa.
