The Treatment Efficacy Of Nerve Block Combined With Gabapentin In The Patients With Post-herpetic Neuralgia

Objective:to measure the effects of paravertebral block and subcutaneous block combine with antiepileptic drug gabapentin complex treatment in the patients with post-herpetic neuralgia(PHN).Method:40 persons suffering from PHN had been selected,selection criteria:①have herpes zoster history;②the skin lesions of herpes zoster(HZ)have completely cured,but the patient still feel pain and uncomfortable.③the course of disease have exceed one 1 month。Exclusion standards:①Haven't have the history and physical sign of HZ;②the diagnosis is unclear.③the person who have severe systematic disease. Randomly divide the 40 persons into two groups(A and B),20 person each group. The group A is treat with nerve block,the mean age is 66.66,using composition of 2%lidocaine+Triamcinolone Acetonide+ Methycobal injection 1000μg+ 0.9%NaCl injection,to treat with paravertebral and subcutaneous block or trigeminal nerve block;The group B is treat with nerve block combined with gabapentin,the methods of nerve block is same to group A,the dosage regimens of gabapentin begin with initial dose,300mg Qd in the first day,300mg Bid in the second day,300mg Tid in the third day,then maintain the dose till the pain have disappear or relief.Each group treats once weakly,and accepts four time treatment.The effects were evaluated by VAS,VRS,BRS-S and sleeping time in 24h before erery treatments,and observe the side effects and complication. Results:Before the treatment,the VAS,VRS,BRS-S and sleeping time in 24h of two groups haven no significant difference(P＞0.05),while after the treatment, the pain scores of two groups are different(P＜0.05).The scores after treatment decrease with time comparing with that before treatment within each group(P＜0.05),but B group is more obvious than A group(P＜0.05).All the groups hadn't evident side effects and complications occur.Conclusion:The two methods of treatment have evident effect,but nerve block combine Gabapentin are better than nerve block alone.
