Objective: To study the relationship between polymorphisms of vascular endothelial growth factor (VEGF) gene and diabetic nephropathy in type 2 diabetes mellitus.Subjects and Methods: A total of 157 patients with type 2 diabetes mellitus were recruited for randomly from our apartment .All patients were classified as type 2 diabetes mellitus without complications and type 2 diabetic nephropathy(DN) based on 24h microalbuminuria. 60 healthy controls who had maden health examination in our hospital were recruited.3ml blood was taken from the elbow vein.Human genomic DNA was extracted by UNIQ-10 collumns. Genotyping for the -460 and +405 polymorphisms were carried out by PCR-RFLP analysis(polymerase chain reaction- restriction fragment length polymorphism). Clinical characteristics such as 24h microalbuminuria,HbA1c,BMI,TC,TG,Cr, etc were measured and calculated meanwhile. All data was tested the normality. Chi-square analysis was used to test whether VEGF genotype distribution followed the Hardy-Weinberg equilibrium. Normally distributed data was demonstrated using means±SD( x±s). Measurement data was compared using one-way analysis of variance (ANOVA). Numeration data was compared using Chi-square analysis. Risk factors for type 2 diabetic nephropathy using multiple logistic regression analysis.Results: (1).Type 2 diabetic patients had high HbA1c,BMI,Cr,BUN,TG level than those of normal controls(p<0.01). HbA1c,BMI,TC and BUN levels were higher in paients with DN than those without(p<0.05).(2). VEGF-460 and +405 genotype CC and C allele frequencies were significantly increased in patients with DN than without and controls(p<0.05 or p<0.01).(3). DN morbidity rate was significantly increased in subjects with the VEGF-460 CC genotype compared with CT genotype(60.0% vs 36.7%,p<0.01)and TT genotype(60.0% vs 22.8%,p<0.01). The odds ratio (OR) of DN for the CC genotype to the TT genotype was 5.08.At the same way morbidity rate was higher in subjects with the VEGF+405 CC genotype than CG genotype(62.2% vs 37.0%,p<0.01)and GG genotype(62.2% vs 25.0%,p<0.01).OR for the CC genotype to the TT genotype was 4.93.(4). Logistic regression analysis revealed that HbA1c,BMI,and the VEGF gene polymorphism were risk factors of DN.Conclusion: VEGF genetic polymorphism is probably associated with DN in Type 2 Diabetes mellitus. VEGF-460 and +405 C allele is probably a susceptible allele of type 2 diabetic nephropathy.
