Background and Objective Ovarian carcinoma,one of the most commonmalignancy of women reproductive-tract, threatens women's health. Serousadenocarcinoma of ovary is the most common ovarian cancer with poorprognosis.The 5-year survival rate of patients with advanced stages is only10%-20%. Adrenomedullin (ADM) was a peptide recently discovered,whichhad been implicated as a mediator of several diseases. ADM was expressed in avariety of tumors, had been shown to be a mitogenic factor capable ofstimulating growth of several cancer cell types and a survival factor for certaincancer cells. Hypoxia is a major event that occurs in most solid tumors.Intratumoral hypoxia is sufficient to activate the key transcription factor,hypoxia-inducible factor-1α(HIF-1α) that mediates the activation of the"survival machinery" in cancer cells. HIF-1αis also correlated with increasedangiogenesis, malignant progression, poor patient prognosis and chemoradio-resistance. We investigated the expression of adrenomedullin(ADM) and hypoxiainducible factor-1α(HIF-1α) protein in serosity ovarian tumors and exploretheir relationship with the development and prognosis in serousadenocarcinoma of ovary.Methods Expressions of the ADM and HIF-1αwere detected by immuno-histochemical analysis in 16 normal ovarian tissues,18 serous cystadenoma of ovary,15 boderline serous cystadenoma of ovary and 57 primary serousadenocarcioma of ovary. The protein expression was classified as mild(+),moderate(++) and intense(+++), based on the intensity of immune -staining.Statistical analysis was carried out using SPSS software (version 11.5).Analysis showing a confidence interval of 95%(P<0.05).Results 1. ADM protein expression was localized in the plasma of tumor cellsand higher in tissues of ovarian cancers than those in borderline tumors,cystadenomas and normal ovarian tissues (P<0.05). HIF-1αprotein expressionwas localized in the plasma or nucleus of tumor cells,especially in the plasma.There were no differences of HIF-1αprotein expressions in borderline tumorsand cancers (P>0.05), but higher than those in cystadenomas and normalovarian tissues (P<0.05).2. The expression of ADM and HIF-1αis related to pathological classification,differentiation and lymph node metastasis(P<0.05).3. There was a positive correlation between ADM and HIF-1α.(r_s=0.367,P<0.05).Conclusion ADM and HIF-1αmay play an important role in the occurrenceand progress of serous cancinoma of ovary. ADM and HIF-1αexpression weresignificantly associated with tumor invasion, metastasis and prognosis, sopatients with high-risk factors could be identified according to ADM and HIF-1αexpression level in serous cancinoma of ovary, patients' prognosis could beevaluated, and reasonable treating plans could be made. ADM expression maybe under the regulation of HIF-1α.
