IntroductionProtein energy malnutrition and endosecretory dysfunction are common inchronic renal failure(CRF). Malnutrition has became one of the independentdangerous factors that influence prognosis in CRF. As endosecretory hormantestosterone and leptin have play important roles in the synthesis of protein andfat. and accommodation of malnutrition. But few study of testosterone in CRFhave been done at domestic. We investigate the changes of serum testosterone, leptinin CRF patients. Study the relations between the testosterone and leptin, and theindices of malnutritions. To demonstrate the significance of the changes of serumtestosterone and leptin level in chronic renal failure, and the correlation betweentestosterone and leptin with the indices of malnutrition.Materials and Methods64 patients with chronic renal failure and 30 healthy subjects for control wereenrolled in this study. Serum testosterone and leptin were detected byradioimmunoassay, prealbumin, hemoglobin were examined; and body massindex(BMI), triceps skin-fold (Tsf), arm circumference (Ac), arm muscle circum(Amc) were also measured. CRF patients was classified malnourished subgroup andnon-malnourished subgroup according to Subjective global nutritional (SGA).All of the results were transformed to datas through image analysis system,which were expressed as (?)±S. The valutes were analyzed, using f testing andcorrelations analysis of SPSS software. Results1 Serum testosterone, leptin levelSerum concentration of testosterone in CRF group were significantly lower thanthat of healthy control group, Serum concentration of leptin were significantlyhigher (P<0.05).2 The correlations between serum testosterone, leptin and prealbumin, hemoglobin.The correlations between serum testosterone and leptin.The testosterone level ofthe patients was positively correlated with hemoglobin, and prealbumin(P<0.05).The leptin level was reversely correlated with prealbumin(P<0.05). No obviouscorrelation between testosterone and leptin's level.3 The correlations between serum testosterone, leptin and the Indices ofmalnutrition.The Indices of malnutrition in CRF were lower than that of controlgroup(P<0.05). The serum testosterone of male patient positively correlated withBMI(P<0.05). The level of leptin and testosterone have no obviously correlationbetween TSF, AC, AMC in CRF patients.4 serum testosterone,leptin level in malnourished subgroup and non-malnourishedsubgroupSerum concentration of testosterone in malnourished subgroup weresignificantly lower than that of non-malnourished subgroup(P<0.05),whileSerum concentration of leptin were significantly higher(P<0.05).ConclusionEnd-stage CRF patients usually have hype leptin and low testosterone level,which had relationship with malnutritional status. However, No obvious correlationbetween testosterone and leptin's level.
