Background and Objectives Corneal epithelium is easily damaged by physical/ chemical causes, microorganism, parasite, and other causes, which usually leads to indolence or incoalese, or, in severe conditions, leads to corneal ulcer, corneal perforation, and other complications, and is a delicate problem on clinical treatment. Moreover, quick and total corneal epithelial recovery after ophthalmic surgery and trauma is essential for maintaining normal sight. Although effective corneal epithelial recovery aided by bFGF, EGF, and other medications on clinical applications in recent year is progressive, the effect is still not satisfactory. According to researches, protein-free calf blood extract is a bio-active content extracted from the blood of 1-6 month(s) old calf which has passed quarantine test. It is a cellular respiration activator without protein and antigen. The contents are mainly micromolecular polypeptide, amino acids, nucleotides, and other substances. The pharmacological action is to improve cellular incept and utilization of oxygen and glucose, increase the synthesis of triphosadenine, accelerate the transportation of nutritive substances, and promote tissue recovery and regeneration. The research is to evaluate the clinical effect on treatment of corneal epithelial defect by protein-free calf blood extract eye gel.Methods Parallel comparison method on randomly selected double blind patients under positive medication is adopted. According to conditions of case groups, 100 corneal epithelial defect cases are observed (including punctate defect and lamellar defect), which are randomly divided into two groups treated by protein-free calf blood extract eye gel (50 cases) and bFGF eye drop (50 cases)rescpectively. The gel and bFGF are both applied 4 times/day. Symptoms in the 1st, 3rd, 5th, 7th, 10th, and 14th day after treatment are examed and recorded. Visual ability test, slit lamp microscope, and corneal fluorescein staining test are carried out. Patients of lamellar defect are slit-lamp-microscoped. At any of the exam, if a corneal epithelial defect is cured, or the symptom or the physical sign disappears on patient, the test terminates. The maximum course of treatment is 14 days. Statistics by evaluation at symptom and physical sign before/after treatment will be analyzed so as to obtain therapeutic effectiveness on treatment of corneal epithelial defect by protein-free calf blood extract eye gel.Results1. 25 cases of punctuate defect at corneal epithelium are in the treatment group by protein-free calf blood extract eye gel, among which no case of exfoliation is found, 18 cases are cured, and 3 cases in evident effectiveness. The cure rate is 72%, effective rate 82%, and average days for corneal epithelial recovery are 11.64±2.91d. 25 cases of punctuate defect at corneal epithelium are in the treatment group by bFGF, among which no case of exfoliation is found, 17 cases are cured, and 3 cases in evident effectiveness. The cure rate is 68%, effective rate is 80%, and average days for recovery are 10.92±3.85d.2. 25 cases of lamellar defect at corneal epithelium are in the treatment group by protein-free calf blood extract eye gel, among which 2 case is exfoliation, 19 cases are cured, and 2 cases in evident effectiveness. The cure rate is 82.6%, effective rate 91.3%, and average days for corneal epithelial recovery are 6.68±3.91d. 25 cases of lamellar defect at corneal epithelium are in the treatment group by bFGF, among which 2 cases are exfoliation, 18 cases are cured, and 2 cases in evident effectiveness. The cure rate is 78.26%, effective rate 86.95%, and average days for coneal epithelial recovery are 7.23±2.76d.3. In the cases of punctuate defect and lamellar defect of corneal epithelium treated by the gel,integrals of symptom and physical sign gradually decline during the treatment. By comparison of each therapeutic effectiveness before/after treatment, evident statistical discrepancy shows up (P< 0.05).4. Therapeutic effectiveness on recovery of corneal epithelial defect by protein-free calf blood extract eye gel and bFGF are basically similar, which shows no obvious statistical discrepancy (p>0.05).5. Average days for corneal punctuate defect epithelial recovery are11.64±2.91d , Average days for corneal lamellar defect epithelial recovery are 6.68±3.91d in the treatment group by protein-free calf blood extract eye gel.6. No evident irritation and negative reaction are found in the two groups during treatment. Conclusion1. Protein-free calf blood extract eye gel can effectively aid the recovery of corneal epithelial defect,the therapeutic effectiveness on corneal epithelial defect recovery by protein-free calf blood extract eye gel and bFGF are basically similar.2. Recovery effect on treatment of lamellar defect at corneal epithelium due to traumatic and surgical by protein-free calf blood extract eye gel is better than that caused by epithelial herpes simplex keratitis.3. Protein-free calf blood extract eye gel is a safe eye drop medication.
