A Comparison Of Different Endometrial Pretreatments Prior To TCRE

Objective:To Compare the effects of different endometrial pretreatments prior to TCRE.Methods:A total of 119 patients,who had diagnosed EP by hysteroscope from Jan.2006 to Jan.2007 were followed up.The mean age was 39.26±4.66years(25～49years).All patients will have transcervical resection of polyps(TCRP)with endometrial resection(TCRE).A total of 119 patients were randomly divided into five groups;A group(medical pretreatment);A group was divided into two groups,Algroup (gestrinone)with 32 cases,receiving twice 2.5mg tablets weekly for 4 weeks,followed by TCRP with TCRE.A2group(GnRH-a)with 8 cases,receiving two injections of GnRH-a (3.6 mg subcutaneously)at the first day of menstrualcycle ahd again after 28 days, followed by TCRP with TCRE.A3group(gossypol)with 22 cases,receiving 20mg tablets daily for 30 days;followed by TCRP with TCRE;B group(mechnical pretreatment);27 cases,transcervical resection of polyps with endometrial resection after vacumm curettages.Cgroup,30cases,without any pretreatment before transcervical resection of polyps with endometrial resection.Results:endometrial thickness with A1, A2,A3 and B groups were less than C group(P＜0.05),endometrial thickness with A2 groups was less than A1 andA3groups(P＜0.05).endometrial thickness with A1,A2,A3 and B groups were not significantly different(P＞0.05);The numbers of endometrial glands near the basal layer with A1,A2 and A3 groups were lower than that in the B and C groups(P＜0.05);the stromal components were more compact than the B and C groups (P＜0.05).The numbers of endometrial glands near the basal layer with A2 group were lower than that in the A1 andA3 groups;the stromal components were not more compact than A1 andA3groups(P＜0.05).The numbers of endometrial glands and the stromal components with Band C groups were not significantly different(P＞0.05);Duration of operation with A1,A2 andA3 groups were shorter than that with B and C groups;No menses rates with A1,A2 and A3 groups were significantly higher than that with B and C groups no menses rotes with A1 group(37.5%)was significantly higher than that with A2 (23.3%)and A3(27.3%)groups.Conclusion:Endometrial pretreatment before TCRE will enhance the effects of operation and no menses rates after operation.How to choose different endometrial pretreatment before TCRE? We think that different patients will choose different endometfial pretreatment,only a kind of endometrial pretreatment will not be adopted by all patients.
