Objective: In this study, through the comparison of the concentration of endothelin-1 and heart rate variability in ACS patients and SAP ones, we verified if the concentration of ET-1 was correlated with HRV in ACS patients ,and whether the correlation with the prognosis of ACS patients and SAP ones existed.Methods: One hundred fifty-one patients were selected at random during July, 2005 to November,2006,and divided into five groups, including 31 cases of normal contrast group in healthy people and 30 cases of contrast group of SAP patients as experiment contrast. And the experiment group was further divided into three groups: UAP, STEMI, NSTEMI. All cases were, within 24 hours, checked by DCG and tested the blood serum concentration of ET-1 by ELISA. All cases were checked by coronary arteriography. To follow up all CHD cases ,that was observed the rate of cardiovascular events in the near future. The results were analyzed with the statistics software of SPSS13.0.Results:①The blood serum concentration of ET-1 in STEMI group,UAP group ,NSTEMI group and SAP group were higher than that in the normal contrast group, and the difference was significantly obvious,P<0.01 ; the the blood serum concentration of ET-1 was of great difference between in STEMI group and in NSTEMI group ,UAP group,SAP group,and the difference in each group were significant,p<0.01 ;②HRV was lower one by one in STEMI group ,NSTEMI group,UAP group and SAP group,and compared with each other or normal control group, all of the difference were significantly obvious,P<0.01.③With the increase of the severity of coronary artery lesions, the index of HRV which were reflected the function of sympathetic nervous system: SDNN,SDANN,SDNN Index obviously were lower, but the index of HRV which were reflected the function of parasympathetic nervous system: RMSSD, pNN50 were not obviously changed④The correlation analysis showed that the blood serum concentration of ET-1 and HRV was related negatively.⑤The incidence rate of the acute cardiovascular disease in the near future was higher with high level of blood serum concentration of ET-1 and Low HRV in the ACS patients,and the incidence rate of the acute cardiovascular disease in the near future was correspondence lower with a low blood serum concentration of ET-1 and high HRV in ACS patiens.Conclusion:1.The blood serum concentration of ET-1 could be used to predicting the danger of the acute cardiovascular events in a short term in ACS patients. 2.HRV can be looked as a no-traumatic index to get the message of the severity of coronary artery lesions indirectly,and predict the dangerous of the acute cardiovascular incident in the near future in ACS patients. 3. The level of ET-1 and HRV was negatively related in CHD patients,both of them might be existed a internal connection in the development of ACS.4. There were a higher incidence rate of the of acute cardiovascular incident in the near future in the ACS patients with high level ET-1 and low level HRV. So the two indexes could be used together to predict the prognosis of ACS patiens more efficiently.
