An Analysis On The Current Situation Of The Health Resources And The Countermeasures

Objective: through the analysis of the effect of the population, disease spectrum, and the economic level etc. on the requirement for the health services, to make clear the situation of the collocation of the health resources in Zhejiang Province, in order to provide some suggestions for the regional health program and better the collocation of the health resources.Methods: to collect and make better use of the social basic circumstances such as situation of the administrative area, population distribution and natural alteration, economic development, people living standard, and the major health problems, the circumstances of the collocation of the health resources in Zhejiang Province such as the number of the medical establishments, the number of the sickbeds, the number of the medical technicists etc, and other data such as the health policy of Zhejiang Province etc. The data was analysed with the statistical software of SPSS for Windows Ver12.0.1.Results: (1) population character, economic development standard, and the major health problems of the residents: the population of Zhejiang Province is 45,772.2 thousand, population density is 442 per km~2, and the birth rate and mortality is well controlled, male-female ratio is 1.05:1, 13.81% of the people is above 60 years old, which means a higher population aging; GDP of Zhejiang Province in 2004 was 2,388.653 billion RMB and GDP per capita was 248,691 RMB, pure income of Country dweller per capita was 6,096 RMB and that of urban dweller was 14546 RMB. The Engel's Coefficient had been descending year by year, and the whole society has become a prosperous one. In 2004, the preceding five death causes of diseases were still neoplasm, diseases of respiratory system, cerebrovascular disease, trauma and intoxication, and cardiac disease. During 2003-2004, infectious incidence rose sharply, and enteric infection diseases were well controlled, while the incidence of respiratory infectious diseases, natural focus disease, arthropod-borne infectious diseases, and sexually transmitted diseases had rose significantly. (2) The current situation of the collocation of the health resources in Zhejiang Province: in 2004, there were 11,937 medical establishments in Zhejiang Province, which meant there were 0.26 medical establishments per one thousand people, and less were distributed in some certain regions which had a larger population density. There were 135,139 sickbeds and each one thousand people shared 2.95 sickbeds, while each one thousand country dwellers shared fewer sickbeds than the mean level of the whole country. There were 221,539 medical members totally, and each thousand people shared 4.05 medical technologists, 1.81 physicians and 1.20 nurses or primary nurses. There were 4 cities whose three indexes were all below the mean level of the whole province. And the ratio of clinicians with senior professional titles to middle and to junior was 1:1.6:3. More than 65% of medical members were in the county and upper hospital, and less than 30% were in the health center and ambulant clinic. In the hospital, certified physicians were 58.32%, and certified assistant physicians were 23.84%, certified nurses were 77.54%. In the health center, certified physicians were 18.05%, and certified assistant physicians were 46.86%, certified nurses were 11.04%. Among the fundamental medical treatment facilities whose price was below 500,000 RMB, 72.06% was in the hospitals of counties and cities, and only 13.93% were in the health centers; while among those large-scale medical treatment facilities whose prices were above 500,000 RMB, 83.44% were centered in the hospitals of counties and cities, and 8.11% were in the health centers. (3) Utilization rate of medical resources in Zhejiang Province: for the recent years the usage rate of the sickbeds and the working efficacy has been rising year by year: the usage of the sickbeds of hospitals (87.59%) and the rate of the beds(25.35) were significantly higher than those of the health centers of small towns(32.98%, 19.45), and in 2004 the usage of the sickbeds and the rate of the beds were both below the mean standard of the whole country, and there were certain differences among the different cities.Conclusions: (1) the health situation and the requirement of Zhejiang residents: aging structure of population showed an aging pattern, while population ratio of low age was much high, and educational level was much low. The great changes of the mode of life and the living environment have brought forth the great changes of spectrum of disease and spectrum of death causes, which augmented the consumption of the health resources. All of these raised more requirements on the health services which meant a great challenge. (2) The major problem of the collocation of the health resources in Zhejiang Province: the collocation of the health resources of Zhejiang Province has been increasing for recent years but were left behind by the economic development standard; and the collocation of the health resources was not so rational; functional dislocation was found concerning the medical establishments, and the patients were willing to visit the big hospitals which led to a poor utilization efficiency of basic medical establishments. Countermeasures: (1) to better the collocation of the health resources, and improve the utilization efficiency of the health resources; (2) to adapt to the change of the aging population structure, spectrum of the disease and spectrum of the death cause, and strengthen the prevention and health service; (3) strengthen the health service of the community to shunt the patients properly.
