Objective: To study the significance of trophoblastic antibody(TA-Ab) in diagnosing alloimmune spontaneous abortion and the changesof T-helper 1/T-helper 2 type cytokines in immune spontaneous abortionpatients.Methods: Blood samples were obtained from 47 women withrecurrent spontaneous abortion (study group) and 23 normal womenwithout spontaneous abortion before (control group) for the determinationof serum TA-Ab and concentration of T-helper 1 type cytokine (interferongamma ), T-helper 2 type cytokine (interleukin-4) by enzyme-linkedimmunosorbent assay (ELISA), and the samples of study group weretested by dot-immunogold filtration assay for serum anticardiolipidantibody (ACA) further more. All the samples were withdrawn fromwomen in the first trimester of pregnancy.Results:1.The positive rate of TA-Ab was 27.7% in study group, which wassignificantly higher than that in control group (4.3%) (p<0.05), thepositive rate of ACA was 47% in study group; 2.The percentage ofpatients with single TA-Ab positive, single ACA positive, both TA-Aband ACA positive, or both of them negative in the study group was 15%,34%, 3% and 38%, respectively; 3.Levels of IFN-γwere significantlyhigher in TA-Ab positive with or without ACA positive group than inTA-Ab negtive with or without ACA positive group and control group(p<0.05); 4.Levels of IL-4 were significantly higher in ACA positive withor without TA-Ab positive group than in ACA negtive with or withoutTA-Ab positive group and control group (p<0.05); Levels of IL-4 in the control group were significantly higher than in ACA negtive with orwithout TA-Ab positive group (p<0.05); 5.The ratio of Th1/Th2 wassignificantly higher in the TA-Ab positive ACA negtive group than inother groups, and it was significantly lower in TA-Ab negtive ACApositive group than in other groups; There was no significant differenceof Th1/Th2 ratio among control group and groups with both TA-Ab andACA are positive or negative.Conclusions:1. The positive rate of trophoblastic antibody was significantlyincreased in patients with recurrent spontaneous abortion, it can be usedfor assistant diagnose of alloimmune abortion.2. Trophoblastic antibody and autoimmune antibodies may both existin patients with immune disorder abortion.3. Cytokine imbalance exists in patients with alloimune abortion andautoimmune abortion, the formal one is mainly T-helper 1 type cytokine,while the later one is mainly T-helper 2 type cytokine.4. Serum T-helper 1 type and T-helper 2 type cytokines' levels wereboth increasing in patients with immune disorder abortion, while the ratioof Th1/Th2 was not significantly changed. |
