IntroductionP16 is a multipl tumour suppressor gene,which is the first one being abstraction and clone. We can find its deletion and mutation in several kinds of human turners. P16 gene involves into regulation of cell-cycle,It may be the center of cell growth differentiation control.HPV is well known currently as a virus-oncogene.The oncoprotein expressed from HPV can lead to epithelial cell immortalization, cell growth, multiplication dropout, apoptosis abnormal.Studies for breast cancer, esophageal carcinoma and oral cavity carcinomatous found that there was extremely altus positive expression rate of HPV.Especially in the study of cervical cancer,the positive rate is approach 99%.Vulvar malignant tumor agreement account to 1% in all the female malignant tumor,and its incidence rate has a rising tendency in recent years. But the etiological of Vulvar malignant is still not known.This research will detect P16 gene and HPV16 DNA by PCR in normal skin,VIN and vulvar carcinoma to approach thire relationship.Materials and Methods1. Materials and reactant: carcinoma of vulva 37 cases, VINIII 10 cases and nomal Skin of vulva 10 cases were from the First Affiliated Clinical Hospital of China Medical University ,and were confirmed by pathological examination after operation in the Genecology. Dimethyl benzene, dehydrated alcohol, DNA extract kit and primer (from Shanghai Sangon company ),Taq enzyme, DNTP, ddH2O and gelose (from Dalian Takara company).2.DNA extraction is according to the direction of the kit, after that carry out P16, HPV16 andβ-actin PCR amplification,the production will be tested by gelelectrophoresis in 2% agarose gel contained EB. Results1.The positive rate of P16 in normal vulvar skin is 100%,in VINIII is 50%,in vulvar squamous cell (VSCC) is 23.08%,in basal cell carcinoma (BCC) and verrucous carcinoma (VC) is 0.2. The positive rate of HPV16 in normal vulvar skin is 0, in VINIII is 50%, in BCC is 80%, in VC is 66.67%,in VSCC is 0.3. In VSCC, The positive rate of P16 in I -II stage is 27.78% ,in III-IV stage is 12.15% ; in well-differentiated VSCC is 25%, in moderately differentiated VSCC is 28.57% and in poor differentiated is 0 ; without lymphoid node metastasis is 31.25%,with lymphoid node metastasis is 10%;over 50y old is 30%,below 50y old is 18.75%.4.In the HPV16 DNA positive cases of BCC and VC ,without P16 gene is detected; in the HPV16 negative VSCC cases,the positive of P16 is 6 cases.Conclusion1 .The devitalization of P16 gene has a close relationship with the carcinogenesis and growth of vulvar cancer, has a direct correlation with clinical stage,dedifferentiation and lymphoid node metastasis.2.HPV16 has a close relationship with the carcinogenesis of VINIII,BCCand VC, has not a close relationship with VSCC.3.The infection of HPV disturbed the function of P16,both of them have joint action in carcinogenesis of BCC and VC.
