Abstract/Summary:
Objective: This study was performed to test the diagnostic value of anewly developed enzyme-linked immunosorbent assay (ELISA) for thedetection of antibodies against a modified citrullinated vimentin(anti-MCV) in comparison with the anti-cyclic citrullinated peptidesELISA test system and IgM-RF in patients with rheumatoid arthritis. Thediagnostic significance and the prognostic value of the anti-MCV test wascompared with the commercial anti-CCP ELISA and IgM-RF. Sensitivity,specificity, as well as the positive predictive value (+PV) and thenegative predictive value (-PV) for the differentiation between RA andnon-RA rheumatologic patients were determined using both testsystems. Expecially to ditermin the relationship between anti-MCV anddisease activity score using 28 joints and 3 variables as well as Sharp/vander neijde scores.Methods: Using the serum ELISA testing to detect anti-MCV antibodiesin a group of 52 RA patients including early RA 12 and follow ups. Deseasecontrol group include others connect tissue disease such as Systemic LupusErythematosis 28,,Primary Sjogren's Syndrome 11, Systemic Scleroderma6 and Polymyositis and Dermatomyositis 3. In the same time, we alsocacullate the DAS28-3 scores and Sharp scores in this group.Results: A sensitivity of 78.85% for anti-MCV and 69.23% for anti-CCPwas calculated for the RA patients. The specificity, compared with healthysubjects and patients with other rheumatologic diseases was 93.59% foranti-MCV, 85.90% for anti-CCP, A good correlation of the anti-MCV withthe disease activity score was documented (Spearman's correlationcoefficient 0.504 while there was no correlation between SHARP and theanti-CCP results.Conclusion: With comparable specificity, the anti-MCV assay showed a substantially better sensitivity than anti-CCP. The anti-MCV resultscorrelated, in contrast to the anti-CCP test system, with disease activityand is therefore an ideal basis for an early RA therapy.Due to our recentresults, anti-MCV is a significant better diagnostic and prognostic RAmarker than anti-CCP, also it represents an important parameter formonitoring and diagnosing RA, especially in early cases.
