Clinic Evaluation Of Anti-mutated Citrullinated Vimentin In Rheumatoid Arthritis

Objective:This study is to detection the against mutated citrullinated vimentin (anti-MCV) antibody in diagnosing rheumatoid arthritis, as well as anti MCV and rheumatoid factor (RF) in rheumatoid arthritis (RA) contrast with the diagnosis of sensitivity, specificity, positive and negative predictive values, and the Diagnostic significance of MCV and RF combined monitoring in the diagnosis of rheumatoid arthritis, and exploration the relationship between MCV Patient age, disease duration, duration of morning stiffness with rheumatoid arthritis disease activity index of DAS28integral, tender joint count, number of swollen joints and laboratory indexes of CRP, ESR.Methods:60patients with rheumatoid arthritis were divided into two groups: early rheumatoid arthritis group (12cases), in advanced rheumatoid arthritis group (n=48); selected control group of two groups:connective tissue disease group (10cases), and healthy control group (20cases). Rheumatoid arthritis diagnosis standards are in line with the ACR/EULAR2009years RA diagnostic criteria (score>6) or in line with the ACR-1987standard. Connective tissue disease group were consistent with the corresponding provisions of international standard. The application of enzyme linked immunosorbent assay (ELISA) in serum anti MCV antibody surveillance. The evaluation of contrast,MCV and rheumatoid factor (RF) in rheumatoid arthritis (RA) in the diagnosis of sensitivity, specificity, positive and negative predictive values, and the Diagnostic significance of MCV and RF combined monitoring in the diagnosis of rheumatoid arthritis, and and exploration the relationship between MCV with Patient age, disease duration, duration of morning stiffness.rheumatoid arthritis disease activity index of DAS28integral, tender joint count, number of swollen joints and laboratory indexes of CRP, ESR. Analysis of the method of using SPSS13.0software for statistical processing.Results:(1) anti-MCV, RF in early RA group the positive rate Respectively was75%(9/12),67.7%(8/12), in the late group RA positive rates Respectively were93.7%(45/48),90%(44/48), in other connective tissue disease group the positive rate Respectively was20%(2/10),40%(4/10), in the healthy control group Respectively were0%,10%. Through statistical analysis of early and late RA group and other connective tissue disease group had significant difference (P<0.05).(2) the anti-MCV in rheumatoid arthritis, connective tissue disease group, group of healthy controls the average concentration Respectively were132.5U/ml respectively,11.OU/ml,8.5U/ml, three groups had significant difference ((x=90, P<0.05).(3) the anti-MCV in diagnosis of rheumatoid arthritis in the sensitivity, specificity, positive predictive value, and negative predictive value Respectively were90%,93.3%,98.2%,84.7%. RF in the diagnosis of rheumatoid arthritis of the sensitivity, specificity, positive predictive value, and negative predictive value Respectively were86.7%,80%,93.1%,75%. Anti-MCV combined with RF in diagnosis of rheumatoid arthritis in the sensitivity, specificity, positive predictive value, and negative predictive value Respectively were83.8%,96.7%,98%,74.3%; anti-MCV’s sensitivity and specificity were higher than those of RF. Anti-MCV combined with RF specificity was higher than that of RF, but the sensitivity is lower than RF.(4) anti-MCV have the correlated with RF, CRP, ESR, tender joint count, number of swollen joints, DAS28. anti-MCV has the concentration correlation with disease activity.anti-MCV have no correlated with Patient age, disease duration, duration of morning stiffness. Conclusion:(1) serum index of anti-MCV in diagnosing theumatoid arthritis in relatively independent of rheumatoid factor, and has higher sensitivity, specificity, positive predictive value, and negative predictive value. anti MCV In early diagnosis of RA Can be used as a serological markers. And combined with RF RA can improve diagnostic specificity and efficiency.(2) the anti-MCV and RF has positive correlation.(3) the anti-MCV and DAS28has positive correlation.(4) anti-MCV have the correlation with CRP, ESR, joint swelling, tender joint counts are related, anti-MCV antibody has the concentration correlation with disease activity. anti-MCV have no correlated with Patient age, disease duration, duration of morning stiffness.
