Objective To investigate the serum levels of tumor necrosis factor alpha (YNF-α),interleukin-1β(IL-1β)and matrix metalloproteinasr-9(MMP-9)in the patients with chronic heart failure(CHF)and their relations to heart dysfunction.Methods Forty-eight CHF patients and normal control subjects were enrolled.Serum levels of MMP-9 and IL-1βwere measured by enzyme linked immunosorbenl assays,TNF-αwas measured by specific radioimmunoassay,and echocardiographic studies were performed.Results Baseline serum levels of TNF-α,MMP-gand IL-1βwere increased significantly in CHF patients as compared with normal control group(P<0.05)and were parallel with the classification of NYHA for heart dysfunction.Serum levels of the parameters were more increased in the patients with systolic heart failure than in the patients with diastolic heart failure. Serum TNF-αwas positively correlated with MMP-9 and IL-1β,and was negatively correlated with the value of EF.Serum IL-1βwas positively correlated with MMP-9 and TNF-α,and was negatively correlated with the value of EF.Serum TNF-α,IL-1βand MMP-9 were all positively correlated with left ventricular end diastolic diameter(LVEDd),and were all negatively correlated with the value of EF.Conclusion TNF-α,IL-1βand MMP-9 may be involved in the development of myocardial remodeling secondary to chronic heart failure especially systolic heart failure.They were specific targets for reversing myocardial remodeling,improving heart function and delaying the development of heart failure.Serum levels of TNF-αand[L-1βwere increased significantly in CHF patients as compared with normal control group,and followed by significantly increasing of myocardial fibrosis MMP-9.They were all closely correlated with the level of heart function.To some extent,they can predict the level of myocardial fibrosis.
