Objective To test the expression of P-glucoprotein 170 (P-gp170) in renal tissues of children with primary nephrotic syndrome(PNS), and to investigate the possible role of P-gp 170 in childrenwith glucocorticoid-resistant nephrotic syndrome.Methods 30 children with PNS were divided into two groupsdepended on its effect to glucocorticoid therapy. Steroid-resistantnephrotic syndrome (SRNS) was definited as that urine protein wasnot negative after 8 weeks therapy of prednisone (60mg/m~2 per dayor 1.5~2mg/kg per day, and total prednisone dose≤60mg/d), whilethe steroid-sensitivity nephrotic syndrome (SSNS) was definited asthat urine protein was negative after 8 weeks therapy of prednisonein the same dose above. Renal tissue samples were obtained by renalbiopsy in each of PNS groups. Then immunohistochemistry methodwas used to check the expression of P-glucoprotein 170 in renaltissue of PNS. In each renal section, we selected two proximalconvoluted tubule regions and calculated relative optical density(ROD) of P-glucoprotein 170 expression in proximal convolutedtubule epithelial cells.Results①The cases of SRNS group and SSNS group havenot statistics difference in sex,age,course of disease and renalpathological types;②P-gp170 is mainly expressed in proximalconvoluted tubule regions in renal tissue.③The average ROD valueof P-gp170 expression in SRNS group, SSNS group and normalgroup are 0.0526±0.0046, 0.0327±0.0053, and 0.0345±0.0062 respectively. The average ROD value of P-gp170 expression inSRNS group is obviously higher than that in SSNS group andnormal group, and there is significant difference of P-gpl70expression between SRNS group and SSNS group (P=0.01), whilethat is no significant difference between SSNS group and normalgroup (P>0.05).Conclusions①The expression of P-gp 170 in renal tissues inSRNS group is higher than that in SSNS group.②It indicated thatthe level of P-gp 170 in renal tissue might be a indicator ofglucocorticoid resistance in PNS in children.
