Objective: To investigate the level of nurse's job burnout and the condition of professional practice environment in Changsha; and to explore factors in nurse's professional practice environment which lead to burnout and provide preliminary measures against nurse's job burnout.Methods: 536 nurses in hospital of Changsha attend our investigation from October 2005 to January by stratified randomized cluster sample method. The Maslach Burnout Inventory-Human Service Survey (MBI-SS), Professional Practice Environment (PPE) Scale and Individual Information Sheet was used in our investigation. The internal consistency of PPE was examined and resulted in Cronbach'sα0.887.The content validity index of the scale was 0.82. Descriptive analysis, Wilcoxon, Kruskal-Wallis, Nemmenyi, Spearman correlation analysis, multiple linear stepwise regression were performed by statistics product and service solutions (SPSS) 13.0.Result: 1. The t-test showed our outcome was difference between the average score of depersonalization and ineffectiveness form the norm by Maslach. 2. Nurses in medicine ward had higher level of emotion exhaustion than those in other wards. Nurses in operating room had lower level of depersonalization than those in other wards. Nurses who were 21to 30 years old or had work as nurses for less than 15 years had higher lever of depersonalization. Nurses in rank A hospital of grade 2 had higher level of emotion exhaustion than those in rank A hospital of grade3. 3. The scores of "Control over Practice" subscale were the lowest in 8 subscales of PPE. The scores of "Teamwork" were lower in 8 subscales of PPE. 4. The influential factors of emotion exhaustion were Internal Work Motivation, Control over Practice, Teamwork. The influential factors of depersonalization were Teamwork, Internal Work Motivation, Control over Practice. The influential factors of ineffectiveness were Control over Practice, Internal Work Motivation, Cultural Sensitivity, Leadership and Autonomy.Conclusion: nurse's burnout was in the middle level. The score of emotion exhaustion was in the middle level; the score of depersonalization was in the low level; the score of ineffectiveness was in the high level. The important objects against burnout are medicine nurses, nurses of 21-30 years old, nurses who had worked for less than 15 years. The intervention to burnout can begin with improving nurse's professional work environment and take measures as follows: Periodically measuring and improving nurse's job satisfaction, raising nursing manager's ability and improve their work method, strengthening nurse's control over practice and autonomy, stirring up the nurse's internalwork motivation, building up efficient teamwork.
