Background: Primary liver cancer and colorectal cancer is now the major diseases that seriously endangering human lives and health, ranking the highest cancer mortality rate of 2nd and 4th. Although the improving surgical and interventional techniques extend the survival Time of some liver cancer and liver metastases of colorectal cancer patient, most diffuse tumor or liver dysfunction patients treatment is still unsatisfactory as the majority of liver radiotherapy and chemotherapy is not sensitive. Chinese medicine-based multimodality therapy program shows its unique advantages.Purpose: To observe the therapeutic effect, and the synergies attenuated role in the intervention or ablation therapy, in stageⅣprimary liver cancer or liver metastases of colorectal cancer patients treated by Xiaochaihu Decoction with Huaier-granula.Objects and Methods: A Case Study was performed. Between January 2005 and May 2007, 43 inpatients and outpatients of primary liver cancer and liver metastases of colorectal cancer were enrolled, who were seen in Guangzhou University of Traditional Chinese Medicine. There were no Surgical indications in those patients. With combination of syndrome differentiation and disease differentiation treament, Xiaochaihu Decoction with Huaier-granula were used according to disease differentiation treatment, and Xiaochaihu Decoction was modified according to four patterns: dampness-heat of liver and gallbladder, Liver-Heat and Blood-Stasis, weak spleen and strong liver functions, and yin deficiency of liver and kidney as syndrome diffentiation treatment.Treatment period≥3 months. Primary destination indicator is survival, secondary indicators include tumor response rate, symptoms improvement, weight change and liver function improvement.Results: The overall median survival time(MST) was 293 days(9.8 months), 6-month and 1-year survival rate was 60.2% and 40.3%,respectively. Thereinto, MST was 165 days (5.5 months) vs 300 days (10 months),6-month and 1-year survival rate was 48.3% vs 80.4% and 33.9% vs 41.1% for primary liver cancer and liver metastases from colorectal cancer. In patients with treatment of TCM, MST was 141 days(4.7 months) vs 293 days (9.8 months),6-month and 1-year survival rate was 20.2% vs 73.3% and 13.5% vs 33% for primary liver cancer and liver metastasis colorectal carcinoma. In patients with treatment of TCM combined with interventional therapy or ablation, MST was 537 days (17.9 months), 6-month and 1-year survival rates was 78.8% and 67.5% for Primary liver cancer. Analysising the relationship between liver function and survival time with the single-factor analysis, the results showed that the liver function Grade A, B and C with a median survival time and 1-year survival rate of 537 days (17.9 months), 236 days (7.9 months), 132 days (4.4 months) and 61.5%,27.2%,14.3%.The overall tumor response rate was 20.69% and total stable rate was 72.41% for total cases which could be evaluated, thereinto, it was 17.65,52.94% for TCM treatment and 25%, 100% for TCM combined with interventional therapy or ablation.Chinese medicine can improve the KPS score, Compared with pre-treatment, the difference was significant. TCM can improve Liver function Compared with pre-treatment, the difference was significant. Combining traditional Chinese and western medicine treatment helps maintain the stability of body weight, compared with the pre-treatment, the difference of weight loss was not significant. TCM treatment shows great advantage in symptoms of fatigue, swelling in the abdomen, fever, bitterness in the mouth, thirst, and poor appetite.Conclusions: 1. Chinese medicine treatment can extend survival time of stageⅣprimary liver cancer and increase the survival rate, combining with TACE and radiofrequency ablation therapy, it may further extend the survival time of advanced stage liver cancer patients. 2. Child-Pugh classification of liver function is an important prognostic factor for liver cancer. 3. TCM treatment can improve the KPS score and the liver function of advanced stage liver cancer patient, improve the tumor-related clinical symptoms, reduce the damage to liver function caused by Invasive treatment such as intervention and ablation.
