Objective: To explore the clinical characteristics and the prognosis ofsevere hepatitis. To study the therapeutic efficacy and the act mechanism ofcoolinging blood circulation for removing blood stasis and catharsis. Toinvestigate the clinical significance of this therapy for severe hepatitisand for its deuteropathy. To seek the better strategy against severe hepatitis.Methods: A retrospective controlled study was conducted on 28 patients withsevere hepatitis from the first hospital pertain to guangzhou university ofchinese medicine and beijing youan hospital, which were randomly divided intothe therapeutic group and the control group by means of envelop. At the sametime, therapeutic group accepted traditional Chinese medical treatment ofcoolinging blood circulation for removing blood stasis, clearing heat-toxin,detoxification and treated with retention-enema with Chinese medicine, everyday a dose, all for two weeks, the control group used western medical therapyand artificial liver alone. To analysis the synthetic therapeutic efficacyof these two groups, the period of the disappearance of the main clinicalsymptoms and the decrease of jaundice of the effective cases, and theoccurrence and the therapeutic efficacy of deuteropathy. On the first day ofthe therapy, the serum level of liver function, ETM, TNF-αand IL-1βweredetected respectively, After therapy,we would compare the clinical effectbetween two groups in term of the above content.Results: (1)There was no significant difference in therapeuticeffectivenness between treatment group and control group. However, thetherapeutic effectiveness for relivering syndromes of tradition Chinesemedicine of treament group was superior to that of control group(P<0.05).The rate for improving hypochondriac distress and pain, abdominal distension,acratia and loose stool of treatment group was superior to that of control group(P<0.05). (2)Compared with before treament, the level of TBIL in bothtreament group and control group had been greatly decreased after treatment,at the same time, the level of APTT in both treament group and control grouphad been greatly increased after treatment(P<0.05). After treatment, levelof NH3 of treatment group was lower than that of control group(P<0.05). (3)In treatment group, the levels of ETM and TNF-αhad been obviously decreaedafter treatment(P<0.05). But there was no significant decrease in controlgroup(P>0.05). After treatment, level of IL-1βof treatment group was lowerto that of control group(P>0.05).Conclusion: Severe hepatitis has severe liver impairment with many othersymptoms due to multi-systemic damage out of liver. According to our findings,we may reasonably draw a conclusion that combination therapy with traditionalChinese medical treatment of coolinging blood circulation for removing bloodstasis and catharsis is an effective prescription for treating severehepatitis. It is superior to control group especially in relieving clinicalsymptoms and improving syndromes of traditional Chinese medcine. This clinicalstudy suggests that Chinese medicine may play an important role in inhibitionof ETM, suppressing secretion of TNF-αand NH3, in protection of livercells, decreasing the complication and improving syndromes of traditionalChinese medcine.
