Experimental Study On Zhuling Decoction Combined With Nourishing Yin And Tonifying Kidney Herbs Treating Mesangial Proliferative Glomerulonephritis

Purpose:This research was designed to observe the effect of Zhuling Decoction combined with nourishing yin and tonifying kidney herbs on mesangial proliferative glomerulonephritis by the animal experiments. Discussing the mechanism of Zhuling Decoction combined with nourishing yin and tonifying kidney herbs in treatment of mesangial proliferative glomerulonephritis in the molecule and cell level. Zhuling Decoction combined with nourishing yin and tonifying kidney herbs to provide a better experimental basis for clinical application.Method:40 clean-grade Wistar rats, at the age of 7 weeks, males:females=1:1.they were randomly divided into blank control group (GroupⅠ), Pathologic model group (GroupⅡ), Zhuling Decoction Group (GroupⅢ) and Zhuling Decoction combined with nourishing yin and tonifying kidney herbs group (GroupⅣ) of 10 rats each. Using the improved rat model of Zhang Wuxing's, etc to modelling. Compared to each other after treatment, observed the effect of Zhuling Decoction combined with nourishing yin and tonifying kidney herbs on proteinuria in 24 hours, urinary occult blood, serum creatinine, urea nitrogen, renal pathological structure,α-SMA and TGF-β1 of the groupⅣrats.Results:1. Compared with blank control group, pathologic model group mesangial cells proliferated significantly, glomerular diameter significantly increased; red blood cells and urine protein infiltrated obviously, and most of red blood cells were polygonal. Pathological changes of glomerular basement membrane were obviously. Urine protein was significantly increased. glomerular filtration membrane damaged significantly. A significant increase in serum creatinine, Glomerular filtration rate had declined. Renal function were impaired severely.α-SMA, TGF-β1 were markedly higher than the blank control group. Renal fibrosis occurred. All these indicated that the modeling of rat mesangial proliferative glomerulonephritis model was successful.2. Proteinuria and urinary red cells changed:Proteinuria and urinary red cells of Zhuling Decoction combined with nourishing yin and tonifying kidney herbs group were decreased, indicating that Zhuling Decoction combined with nourishing yin and tonifying kidney herbs can significantly reduced proteinuria and urinary red cells of mesangial proliferative glomerulonephritis rats' and eased glomerular lesions, and relieved symptoms of urine protein. although Zhuling Decoction can reduce proteinuria and urinary red cells, they were significantly different (P<0.05).3. Serum creatinine and urea nitrogen changed:serum creatinine and urea nitrogen of GroupⅣwere significantly decreased, indicating that Zhuling Decoction combined with nourishing yin and tonifying kidney herbs can improve significantly glomerular filtration rate and improve renal functions. Though Zhuling Decoction can reduce serum creatinine, urea nitrogen(P<0.01), they were markedly different (P<0.05).4.Pathological changes:Zhuling Decoction combined with nourishing yin and tonifying kidney herbs group's sacculus of glomerulus opened, mesangial cell decreased; several glomerulus developed adanced fibrosis. Compared GroupIV with GroupⅢ, there were significant difference between two groups (P<0.05)5.α-SMA, TGF-β1 changed:α-SMA, TGF-β1 of GroupⅣwere significantly lower than groupⅡ, indicating Zhuling Decoction combined with nourishing yin and tonifying kidney herbs has anti-fibrosis activity.compared to GroupⅡ, there were significant difference between the two groups (P<0.05).Conclusion:1. Zhuling Decoction combined with nourishing yin and tonifying kidney herbs not only can inhibitting proliferation of the mesangial cells, decreasing serum creatinine, urea nitrogen, athletic hematuria and proteinuria. and easing lesion of renal function, but also can effectively protecting the renal function, improving glomerular filtration rate and preventing the occurrence of azotemia.2. Zhuling Decoction combined with nourishing yin and tonifying kidney herbs can effectively inhibit broadening of mesangial area which caused the addition of the secretion of mesangial matrix.This might be one of the mechanisms.3. the curative effect of Zhuling Decoction combined with nourishing yin and tonifying kidney herbs was better than Zhuling Decoction.
