Thyroid cancer is the familiar tumor of the endocrine system, its incidence rate has been increasing year by year. But the thyroid cancer have no obviously clinical symptom, it is difficult in making a definite diagnosis before the operation.Most of thyroid cancer are well-differentiated and have better prognosis.But thyroid cancer much need reoperation because the misdiagnosis before the first operation and not appropriate surgical operation.The identifying diagnosis of thyroid nodules before operation is the key of treatment. In the meantime the diagnosis of recurrence and metabasis of thyroid cancer important in settling reoperation treatment.Analysing 99mTc-MIBI imaging with the other clinical checks,we studied the clinical value of 99mTc-MIBI imaging in the diagnosis of thyroid cancerObjectives:Studing the clinical value of 99mTc-MIBI imaging in the diagnosis of thyroid cancer and the clinical value in identifying residual focus,recurrencer and lymphatic metastasis after the first operation.Methods:Through the clinical check-up and the ultrasound,45 patients of suspective thyroid cancer and 28 patients of needing reoperation appled 99mTc-MIBI imaging.In the meantime, CT check and Tg measurement were made and check against as a result with pathology.Results: With 99mTcO4- imaging in the diagnosis of thyroid cancer, sensibility was 90.24%(37/41), specificity 23.53%;(13/17). With 99mTc-MIBI imaging in the diagnosis of thyroid cancer, sensibility was 51.22%(21/41), specificity 88.24%;(15/17).Among 28 patient of thyroid cancers needing reoperation,7 patient remained 7 spots,5 spots of 5 patients attending clear manifestation with 99mTc-MIBI imaging;6 patients recurrented in 7 spots. 6 spots of 6 patients attending clear manifestation with 99mTc-MIBI imaging;7 patients had lymphatic metastasis in 12 spots,11 spots in 7 patients attendingclear manifestation with 99mTc-MIBI imaging and 5 patients being confirmed by the pathology.Conclusion: 99mTc-MIBI imaging was important in the diagnosis of thyroid cancer,particularly in identifying residual focus,recurrencer and lymphatic metastasis after the first operation.we can draw the conclusion:1.The suspective patients of thyroid cancer through clinical check-up and Ultrasound got imaging with 99mTc. With cold nodule as the standard,sensibility was 90.24%, specificity 23.53%,and sensibility was higher.2.The suspective patients of thyroid cancer through clinical check-up and Ultrasound got imaging with 99mTc-MIBI.With the cold nodule being filled as the standard, sensibility was 51.22%, specificity 88.24%,and specificity was higher.3.99mTc-MIBI imaging was significative in identifying residual focus and recurrencer after the first operation.4.The value of 99mTc-MIBI imaging was lower in diagnosis of lymphatic metastasis.5.99mTc-MIBI imaging was important in the choice of reoperation and the range of operationg.6.To a certain degree 99mTc-MIBI imaging had false positive and false negative.With combining the other clinical checks, the accuracy of diagnosis can be raised.
