Adrenal tumour is one kind of common tumours of the urinary system. The diagnostic technique of the adrenal tumours is gradually enhanced by the widely using of CT and MRI. More and more incidental adrenal tumours without clinical symptoms are being diagnosed. Primary hyperaldosteronism, Cushing's syndrome, pheochromocytoma and adrenal medulla hyperplasia are the most common ones of adrenal tumours. Immediate and effective surgeries are the main therapy. Open surgeries are comparatively simple and remain to be the most widely used method of therapy in less technically developed hospitals. Open adrenalectomy is considered to be a safe and useful method of therapy, but it brings more injury, more loss of blood, longer recovering time and more possible complications. La paroscopic adrenalectomy now is the standard method of adrenalectomy. Compared with open adrenalectomy, laparoscopic adrenalectomy brings less pain, earlier GI recover, shorter hospital stay and earlier fully recover.Objective To compare and evaluate the clinical value between retroperitoneal laparoscopic adrenalectomy and open adrenalectomy for the treatment of adrenal tumours. Medthods From January 2003 to November 2006, retroperitoneal laparoscopic adrenalectomy (RLA) was performed for 40 cases, 38 cases were successful (Group A), during the same period 41 patients with adrenal tumurs under 6 cm in diameter underwent open adrenalectomy (Group B). The procedure was evaluated and compared in the respects of tumors weight, operation time, blood loss, recovery time of GI function, hospital stay between two groups. Results The mean operating time: group A was 11.8 min longer than group B , mean blood loss in group A was less diminished by 45.2 ml than group B. recovery of GI function ,group A was 1.75 day earlier than group B ,mean hospital stay group A was 2.87 day shortter than group B. Conclusions RLA were better than open method. On the hand of approach to adrenal gland distube to abdominal organ and postoperative recovery on RLA have obvious adventage.
