| Background : Malignant tumor generated from nasal cavity andparanasal sinuses is some common in otolaryngology. Nasal malignant tumor comprise of 2.05-3.66 percent of all malignancies, and 11.9 percent of head and neck malignancies. Carcinomas are much more than tumor carneus, and squamous cell carcinomas is the most common species, comprise of 55. 3 percent of malignancies of nasal cavity and paranasal sinuses, more arising from nasal cavity and maxillary sinus. The generation of nasal malignancies may have relations with factors as following: repeatedly inflamm stimulus, long-term exposure of blastomogens, canceration of benign tumors, surgical trauma and so on. Much more scholars deem that the generation of malignancies is a long-standing and grading procedure resulting from multiple-gene mutation and multiple-factor participation. But the molecular biology pathogenesy of nasal squamous cell carcinomas is not clear at present. There is investigation indicated that EGFR, C-erbB2 in epidermal growth factor receptor family may be have some effect in the generation of larynx, hypopharynx and glossal squamous cell carcinomas. This research will utilize immunohistochemical method to detect the expressions of EGFR and C-erbB2 in squamous cell carcinomas in nasal cavity and paranasal sinuses. Investigating these molecular biological markers and combining with analysis of pathological characters, may help us to select therapy methods and to estimate the prognosis towards patients with nasal squamous cell carcinomas, it has very significant value.Object1. This research is expected to detect and compare the expression and value of epidermal growth factor receptor , C-erbB2 and Ki-67 in squamous cell carcinomas of nasal cavity and paranasal sinuses , nasal inverted papilloma and nasal polyp tissue, and analyses their expression in different groups of pathological differentiation of nasal squamous cell carcinomas, in order to explore the molecular biological effects in the generation and development of nasal squamous cell carcinomas of these biological markers.2. To investigate and compare the expressions and significances of epidermal growth factor receptor and Ki-67 in nasal polyp, nasal inverted papilloma and squamous cell carcinomas which aggravating from nasal inverted papilloma via this research , we expect to explor significances of these biological markers in generation and aggravation of nasal inverted papilloma, and therefore make a guidance for clinical therapy.Method :The expression conditon of epidermal growth factor receptor, C-erbB2 and Ki-67 were examined by immunohistochemical S-P staining method in 12 cases of nasal polyp tissue , 17 cases of nasal inverted papiloma , 31 cases of patients with squamous cell carcinomas of nasal cavity and paranasl sinuses(7 cases of well differentiated, 6 cases of moderately differentiated and 18 cases of poorly differentiated), and among that there are 7 cases that aggravated from nasal inverted papilloma. All statistics data from the research were analyzed by Chi-square test and analysis of variance with SPSS 13.0 statistics software. Result1. The expression and comparison of epidermal growth factor receptor, C-erbB2 and Ki-67 in squamous cell carcinomas of nasal cavity and paranasal sinuses , nasal inverted papilloma and nasal polyp as follows: the over-expression positive ratio of EGFR was 48.93%, 11.76%, 8.33% respectively. There was significant difference of over-expressive intensity among these three groups(P=0.006). Over-expression positive ratio of C-erbB2 was 25. 81%, 5. 88%, 0.00% respectively. Over-expressive intensity disparity has statistical significance among three groups (P=0.048). The mean of positive cells labeling index of Ki-67 was 35. 399%, 16.874%, 7. 651% respectively. The positive expression disparity between groups has significance(P<0.001).2. The group of squamous cell carcinomas of nasal cavity and paranasal sinuses was divided into three groups: well differentiated, moderately differentiated, poorly differentiated. The expression and comparison of epidermal growth factor receptor as follows: the over-expression positive ratio of EGFR was 14.29%, 33.33%, 66.67% respectively. There was no significant difference between them(P=0.245). The positive expression ratio of C-erbB2 was 14. 29%, 16. 67%, 33. 33% respectively. The result of comparison was as same as EGFR(P=0.527).And the mean of labeling index of positive cells of Ki-67 was 33. 203%, 34. 037%, 36. 707% respectively. The disparity of positive intensity has no sifnificance between them (P=0.853).3. The over-expression intensity and comparison of epidermal growth factor receptor and Ki-67 in nasal polyp tissue, nasal inverted papilloma and squamous cell carcinomas which aggravated from nasal inverted papilloma as follows: the positive expression ratio of EGFR was 8.33%, 11.67%, 57. 14% respectively. Over-expression condition between them has significant deviation (P=0. 046). The mean of labeling index of positive cells of Ki-67 was 7.651%, 16.874%, 35.298% respectively, there was significant difference between them(P<0. 001).Conclusion1.As to EGFR and C-erbB2, there maybe have some invariably effect in carcinogenesis and development of squamous cell carcinomas of nasal cavity and paranasal sinuses , and it maybe a considerable event in earlier stage of generic term of squamous cell carcinomas. There positive expression intensity has increasing trend with degrading of cell differentiation degree of the cancers, but the difference has no statistical significance, and therefore they can't be taken as notable markers to measure malignant degree of carcinomas. The marker Ki-67 was taken as a antigen of cell nucleus hyperplasy , it has reflected biological behavior objectively of tumors of nasal cavity and paranasal sinuses, and it has reflected malignant degree of carcinomas to some extent in this research. It can be taken as a objective biologic markers to detect patient's condition and to weight the hazardness of diseases.2. Over-expression of epidermal growth factor receptor maybe a important event in the complex course of nasal inverted papilloma aggradating into squamous cell carcinomas, but it has not conspicuous association with the generation of nasal inverted papilloma in this research. Ki-67 maybe taken as a comparatively reliable nucleus tumor markers to measure activity of cell proliferation. Detecting these two markers united, may help us to measure hazardous degree of nasal inverted papilloma, and to select the modus operandi and treatment prescription. |
