PurposeThis study was to assess in the Early Diagnosis and Treatment of ruptured intracranial aneurysms, the value of CTA, wheather high quality CT angiography can replace DSA or not, wheather the early diagnosis and treatment of spontaneous subarachnoid hemorrhage can only with high quality CT angiography .Methord58 cases of ruptured intracranial aneurysms with spontaneous subarachnoid hemorrhage in 2005.2~2007.2 were diagonsised ,who were all emergency patients, and performed early surgical clipping or coiling with CTA or DSA.SAH (intracranial aneurysms) Emergency Course(Zhejiang University Medical College Srrsh Hospital JCI Clinic Pathway)Results51 cases of ruptured intracranial aneurysms with spontaneous subarachnoid hemorrhage can found aneurisms with CTA. 46 cases were performed operation or coliling in early time.Rechecked by DSA after operation or coiling, It found other unruptured intracranial aneurysms in different places. 7cases found nothing with CTA first, then rechecked by DSA , found 1 case with superior cerebellar artery (SCA) ,other 6 cases found nothing.ConclusionThree-dimensional CT angiography with volume rendering is a quick, reliable, and relative noninvasive diagnostic tool for intracranial aneurysms. It can delineate the aneurysmal morphology in detail, and it can provide enough information for making decisions and planning microsurgical clipping in emergency.In most spontaneous subarachnoid hemorrhage, high quality CT angiography can replace DSA.
