Backgrounds:Total hip arthroplasty is commonly used in hip-joint diseases, with the improvement of artificial joint prosthetic material, surgical instruments as well as joint replacement technique, how to lessen the surgical trauma of THA with qualified surgical effects has become a general concern for orthopedists at present. With the deeper understanding of micro-trauma and the extensive application of micro-trauma techniques, more cases on mini-incision surgery or mini-invasive surgery THA have been reported accordingly, which has gained identification by more and more orthopedist. The application of MIS in THA has opened up a new way for reducing surgical trauma and improving surgically therapeutic effect.Operations trying to lessen the trauma caused by various kinds of total hip arthroplasty are categorized into two main types: minimally invasive surgery which refers to the separation of muscles through one or two smaller incisal openings with articular capsule remained; saturation of articular capsule is an important procedure of this type. Mini-incision surgery refers to the surgery including posterior lateral approach, inferior lateral approach and justofrontal approach. The two types of surgeries have been abbreviated as MIS in most literature. In terms of appearance and applicable prospect, the improvement on this technique has dramatic attractive force. However, compared with other traditional surgeries, the advantages of less hemorrhage, shorter hospitalization and quicker convalescence of MIS are controversial. Experts approving of the advantages believe there's significant improvement in hemorrhage amount, locomotor activity at early stages and hospitalization duration for mini-incision surgery group compared with normal treatment group, the study is lack of strict clinical random pair. Experts with opposite viewsfailed to detect the significant difference in time cost, hemorrhage amount, locomotor activity at early stages and hospitalization duration through retrospective study between the two groups. Furthermore, small incisal opening group is more inferior to normal treatment group in the position of acetabular bone and femoral bone handle prosthesis.Some cases have been selectively performed with posterior lateral mini-incisional total hip arthroplasty surgery and achieved satisfying results, since January, 2005.Objective : posterior lateral mini-incisional total hip arthroplasty surgery was performed on 24 patients and the result was compared with normal treatment group to investigate the indication, feasibility as well as short-term clinical effects of posterior lateral mini-incisional total hip arthroplasty surgery so as to get a more comprehensive and general study on lateral mini-incisional total hip arthroplasty surgery.Method: 24 cases with posterior lateral mini-incisional total hip arthroplasty surgery were compared with normal artificial total hip arthroplasty in incisal length, hemorrhage amount, postoperative drainage, operation time, postoperative pains, hospital days, early locomotor activity, complications and Harri scoring.Results:It is detected that mini-incision surgery reduced operative trauma, operative hemorrhage,postoperative pains,hospital days and promoted early locomotor activity . No impacts on operative time and recent effectiveness have been found..Conclusion:1. Procedures on posterior lateral mini-incisional total hip arthroplasty surgery are comparatively simpler without putting special apparatus in use after mastering surgery of total hip arthroplasty2. It is featured with less trauma, less hemorrhage, easier to accept, feasibility and easier to carry out.3. The indications and complications should be aware of .
