Hypertensive disorders complicating pregnancy is the kind of disease that special occurs during the pregnancy, it threatens the pregnant women and the foetus seriously,it is still the main reason that causes the pregnant women and the newborn to die by far. Eclampsism and eclampsia are the severity stage of it .There are many theories about its aetiology, they reveal its mechanism from different aspects,but have not become unity. It has been conformed that the base pathophysiology is the arteriole spasm of the system, endothelial injury and failure is the key point of the disease, especially of the eclampsism and eclampsia. Based on the study , it has become the hot study in the nation and abroad that body's recovery from the endothelial injury and failure or over-react—-stage of before throm, capillary vessel throm, can make the disease aggravated. For clinic, it has become the important contents that use the calparine to prevent and cure the stage of before throm, capillary vessel throm of the serious eclampsism and eclampsia. von Willbrand Factor ( vWF) is also called factor VIII related antigen ,its increased level is regarded to be one of the markof the endothelial injury and stage of before throm. vWF-A2 is a amino acid sequence that plays the role of function adjustment .It has antigenicity. My study plans to detect vWF—A2 in the blood serum of the patients of eclampsism and eclampsia, the vWF—A2 express intensity and allocation in placenta tissue. To investigate endothelial injury,stage of before throm even capillary vessel throm of the disease.Objective: To detect vWF—A2 in the blood serum of the patients of eclampsism and eclampsia, the vWF—A2 express intensity and allocation in placenta tissue. Then to investigate the role of vWF—A2 in hypertensive disorders complicating pregnancy ,especially in the development of eclampsism and eclampsia. To provide a diagnose evidence for endothelial injury,stage of before throm and capillary vessel throm ,meanwhile give the dependable analysis index for its prevention and cure.Stuff and methods: To collect the serum of 56 preeclampsia and eclampsia women ,32 normal pregnancy women and 21 normal non pregnany women in our maternity department from Oct 2004 to Feb 2007,which subgrouping reference to the diagnosisstandards of hypertensive disorders complicating pregnancy in the sixed edition ?Obstetric & Gynecology? edited by Jieyue. Each groups women have not hypertention,heart disease,kidney disease,diabetes mellitus,liver disease,hemorrhagic disease and thrombus disease,blood transfusion and immunotherapy. Detect the vWF—A2 level in serum with ELISA at Jilin university preclinical medicine college. Mon-antibody is vWF—A2 monoclonal antibody (R&D Biology Reagent Company ).Dis-antibody is caprine anti mice polyclonal antibody signed by horseradish oxidase.Immmunoreagent is the product of Beijing Bingguo Biotechnology Development Concer. Detect the level of vWF—A2 expression and location of 10 normal pregnancy women with S-P immunohistochemistry。Mon-antibody is vWF—A2 monoclonal antibody (R&D Biology Reagent Company )。S-P immunohistochemistry kit,developer DAB kit,polylysine are the products of Fuzhou Maixin Biotechnology Company.The methodS of statistics:Using the CS2000 statistics software,using the gray scale to express the intensity of vWF—A2 masculine,to denote the each groups data using (X±S),comparing each groups data withrank sum test.Result:1. These four groups'serum level of vWF- A2 : the normal late pregnancy team's serum vWF level is higher than the normal no pregnancy team, light eclampsism team is higher than either the normal late pregnancy team or the normal no pregnancy team ,but have no significant difference between these three teams.( P>0.05). vWF level in severe eclampsism and eclampsia in pregnancy is much higher than the front three teams ,and each has significant difference(P<0.01)。2.Four groups'vWF- A2 level in the blood serum,the count of PLT in the blood serum and the relationship between them. About the count of PLT in the blood serum ,normal late pregnancy team are lower than the normal no pregnancy team, light eclampsism team is lower than either the normal late pregnancy team or the normal no pregnancy team ,but have no significant difference between these three teams.( P>0.05).Severe eclampsism and eclampsia in pregnancy is much lower than the front three teams ,and each has significant difference(P<0.01)。it showssignificant negative correlation between four groups'vWF- A2 level and the count of PLT in the blood serum, but only the group of severe eclampsism and eclampsia in pregnancy shows significant negative correlation that the higher the level of vWF- A2 in the blood serum is, the lower the count of PLT in the blood serum is .3.Three groups'vWF—A2 expression intensity and allocation in placenta tissue: They have the same expression allocation, they are the placental villus nutrient cell, endothelial cell of villus capillary vessel, villus interstitial cell and endochylema of decidual cell. The gray scale of the three groups'vWF—A2 expression in placenta tissue-- the normal late pregnancy team is 92.37±18.34, 85.14±15.21 in the group of light eclampsism , and 77.15±10.01 in severe eclampsism and eclampsia. Compared to the front two groups , severe eclampsism and eclampsia's expression rate obviously increased , and has significant difference(P<0.01)。4.The tendency of three groups'vWF—A2 expression intensity in placenta tissue and the count of PLT in the blood serum: The vWF—A2 expression gray scale of placenta tissue decreasesprogressively from normal late pregnancy team,light eclampsism team to severe eclampsism and eclampsia team, that is ,the vWF—A2 expression intensity increases progressively from normal late pregnancy team,light eclampsism team to severe eclampsism and eclampsia team, and have the same tendency in the blood serum.Conclusion:1.It occurs severe endothelial injury,stage of before throm even capillary vessel throm in the system of severe eclampsism and eclampsia patients.2.It occurs severe endothelial injury,stage of before throm even capillary vessel throm in the placenta tissue and villus capillary vessel of severe eclampsism and eclampsia patients.3. The placental villus nutrient cell, endothelial cell of villus capillary vessel, villus interstitial cell and endochylema of decidual cell are the important tissue that synthetize,store vWF for the women of the normal late pregnancy,eclampsism and eclampsia ,they are the important resource of vWF in blood serum during gestation period.4. The notable increase of the level of vWF- A2 in the blood serum and the notable decrease of the count of PLT in the blood serum, and the significant negative correlation between them is either one of important index that diagnose severe endothelial injury,stage of before throm and capillary vessel throm for the patients of eclampsism and eclampsia ,or one of important monitoring index that is helpful to prevent and cure the capillary vessel throm, at the same time, it is also one of convenient,reliable important index that discriminate TTP,ITP and aplastic anemia.
