Research background: The increasing medical cost has become the common problem of the medical insurance all over the world, especially in China. But how to control the medical cost is the key to solve this problem. Currently, people pay more attention on the control of total medical cost, while there is no deep study on disease cost with the health insurance. Shandong Province carried on the basic health insurance for urban workers in 1998 and now it has developed in every way. It is a process to gradually explore and perfect the basic health insurance. After analyzing the data of medical cost of 29 countries, some scholars found that inpatient cost is the major part of the medical cost. As a result to control the medical cost effectively we must firstly control the inpatient cost. In a ear with the medical advancement everyday, how to improve the medical service, decrease medical cost and control the medical expense is the challenge for the government, medical institution and hospital.Objective : To know the effect of the health insurance on the inpatient cost by analyzing the differences of the inpatient cost and the constitutions of the patients with rural health insurance and those at their own cost. To supply reference of the hospital administrators to set up the measures on controlling the inpatient cost and control the increasing inpatient cost of those with rural health insurance.(1) Describe the general status of the patients in hospital.(2) Analyze the distribution and constitution of the inpatient cost of those with rural insurance and those at their cost.(3)Supply advices according to the analysis results.Results:Among all the 1256 cases the married account for 98.20% indicating that there is no age difference in cases. The days in hospital is the main factor influencing the inpatient cost. In the five kinds of diseases the longest time in hospital is 16.21 while the shortest is 9.48. The inpatient cost of those with rural health insurance in coronary heart disease and hysteromyoma are more than that of those at their own cost. Bed cost and nursing cost vary inversely with the days in hospital. Drug cost account for a big part in the total inpatient cost, especially of cerebral infarction. There are great differences among the cost of different diseases. So is the constitution of inpatient cost. While the drug cost is major part in the internal medicine, the treatment cost is the major part of in the surgery. Conclusion and suggestion: (1) Investigate the payment according to the kind of diseases;(2) Limit the cost of diseases in the departments; (3)Improve the The technique level and morals standards of the service.
