Objective To discuss the application of the transabdominal L type incision in the radical nephrectomy.Methods The paper retrospectively analyzed 31 patients operated on radical nephrectomy through the transabdominal L type incision, in department of urology of China Jiapan Union Hospital ,during January 2006 to March 2007.Besids,we let other 36 patients who were operated on through the flank incision during the same period compare with them. Statistical methods were used to analyze the operation time, aerofluxus time, the time of putting out induced draft, inpatient time after operation and operative complications between the two incisions in renal cell carcinomas .Results All operations of the two approaches were successful. The operation time of the transabdominal L type incision is significantly longer than that of the flank incision. As to the aerofluxus time, the time of putting out induced draft and inpatient time after operation, the results of two approaches had nosignificant difference. The rates of operative complications in the two incisions were both fairly low. Among the patients operated on through the transabdominal L type incision, 1 patient had spleen rupture(3.2%), and among the patients operated through the flank incision, 2 patients had pleural injuries(5.9%). There's no si-gnificant difference between the two incisions.Conclusions The transabdominal L type incision has the advantages of good view field, operative safety, thorough lymphadenectomy and small amount of bleeding. It can achieve the same effect as the flank incision in both reducing complications and recovery. Thus the transabdominal L type incisionhas more advantages in the operation of huge tumors .Tumors in any position of kidney can be operated safely, and the spread of tumor cells can be avoided as well. The transabdominal L type incision is an admirable approach of radical nephrectomy.
