Objective: To study the clinical effect on C-irrtable bowel syndrome (IBS-C) treated on-demand with tegaserod combined with medilac-s in improving patients' stool description, clinical symptom and quality of life. Thus to evaluate the efficiency and feasibility of on-demand treatment, And to make sure how to choose a suitable method to treat the IBS-C.Methods: This was a randomized controlled trial.40 patients in QiLu Hospital were divided into two groups, whom were diagnosed IBS-C according to the RomeIII criteria by a simple stool description from October 2006 to January 2007. The two groups were assigned to 8 weeks of different therapies with on-demand treatment: tegaserod , 6mg twice daily ,and medilac-s 500mg, three times daily ,and patients discontinued the drug once symptom abated. Systematical treatment: tegaserod ,6mg twice daily ,and medilac-s 500mg, three times daily ,and patients take the drug continually .Main outcomes assessment included the Bristol Stool Form Scale, the symptomatic scores and a Chinese version of the SF-36 Health Survey Scales at baseline and 8 weeks.Results:8 weeks later, the Bristol Stool Form Scale, the symptomatic scores and the SF-36 Scales at baseline have significant improvement in both groups compared with baseline. But There was no significant difference of efficiency between on-demand treatment and systematical treatment .Conclusion: Tegaserod is a kind of high-selective 5-HT4 receptor partial agonist, which can regulate the motivation of stomach intestine and degrade the visceral hypersensitivity. Tegaserod may be used to treat IBS-C and functional constipation and can improve patients' symptom and quality of life. Medilac-s is made up of Bacillus subtilis and enterococci, which will keep the balance of intestinal mieroecology and also be used to treat IBS. According to the trial, clinical effect on IBS-C treated on-demand with tegaserod combined with medilac-s in improving patients' stool description, clinical symptom and quality of life is as available as that of systematical treatment.
