Objective To summarize the diagnosis and treatment of pancreatic islet cell tumor. Methods Eighteen patients with pancreatic islet cell tumor from 2001 to 2006 admitted in the 2nd Affiliated Hospital of Zhejiang University School of Medicine were analyzed retrospectively. Results Among 18 patients, there were 13 cases classified as insulinoma,4 cases of non-functional pancreatic islet cell tumor, 1 case of gastrinoma. All the patients of insulinoma had Whipple' s triad and the ratio of insulin to glucose was greater than 0.3 mostly. The patients of non-functional pancreatic islet cell tumor had no specific symptoms at the time of diagnosis. The patient of gastrinoma presented with diarrhea. The positive rates of B-ultrasound, CT, MRI and DSA were 83. 3%, 64. 7%, 66. 7% and 100%respectively. 16 cases received operation,15 cases (93.7%) were single, 1 case (6.3%) were multiple. In the 15 cases of single tumor,7 cases were located in the head of the pancreas, 3 cases in the body, 5 cases in the tail. Partial resection with surrounding normal pancreas, distal pancreatic resection with splenectomy and pancreatico-duodenal resection (Whipple' s operation) were the main surgical procedures. The main complications of surgery were pancreatic fistula, haemorrhage and infection of incision. Conclusion Insulinoma and non-functional pancreatic islet cell tumor are the mostly dominant in pancreatic islet cell tumor. It is important to localize pancreatic islet cell tumor preoperatively. Surgery is the only curative method. It has a better prognosis than other abdominal organ tumors.
