OBJECT To study the curative effect of the section neoplasm in fourth ventricle through cerebellomedullary fissure approach, and discuss the main point of the operation. METHODS 18 cases fourth ventricle tumors that have been operatedthrough cerebellomedullary fissure approach in our department from January 2000 to December 2006 were analyzed retrospectively. The complication and prognosis were concluded.RESULT Cerebral aqueduct obstruction were solved in all 18 patients, 14cases underwent total resection and 4 cases subtotal resection. Total resection rate were 77.8 %. All patients were conscious after surgery ,nodead. Tumor reoccur in 1 patients, 3 patients suffered postoperative hydrocephalus and were cured by ventriculoperitoneal shunt.No approach—related complications such as cerebellar mutism, injury of posterior cranial nerves and hemiplegia occurred. CONCLUSIONS During the section neoplasm in fourth ventricle through cerebellomedullary fissure approach, the inferior vermis is not incised, and the cerebellum tissue is not injuried. The anatomical spaces are very enough .The injury and complications are reduced after the operation. The approach is safe and efficacious.
