Objective : In human in-vitro fertilization-embryo transfer( IVF- ET) techniques, how to choose high quality embryo to transplant and reducing the rate of multiple births at the same time does not affecting the pregnancy rate has been concerned by assisted reproductive areas publicly . At present, more commonly used in the screening of embryos morphology indicators there is a certain limitations, therefore explore various scholars from some embryonic development and pregnancy related factors, such as egg shape, prokaryotic period scoring pronged attempt to have achieved some results. In recent years, the relationship between the correlation factor of implanta- tion and embryo quality has aroused the interest of assisted reproductive workers,that is a better reflection of the quality of embryos predictors. Matrix metalloproteinases (matrix metalloproteinases, MMPs), as a protein enzyme to hydrolysis extracellular matrix components, broad participation of implantation embryo and placenta formation process. Among them,gelati- nase B (MMP-9) expression in specific ovarian granulosa cells and embryo implantation of trophoblast cells and endometrial local epithelial cells, mainly involved in the regulation of cell implantation process of the degradation of extracellular matrix.Because MMP - 9 is mammalian reproductive implantation process hydrolysis of extracellular matrix of the most important proteolytic enzyme, it has become the hottest international research on the implantation relevant factor. The purpose of this study is to analyze the relationship between MMP9 secretion in follicular fluid and embryo culture medium and embryonic development,and the effect toIVF-ET outcome.Methods: In this paper, the objects are women patients:74 cases, who from August 2006 to April 2007 attendance at the Maternity Hospital of Dalian reproductive endocrinologist, only by bilateral tubal occlusion and the exclusion of other factors infertility (male factors, endometriosis and PCOS)and application IVF-ET with long line programs ovulation. In this study,by MMP9 single clonal antibody as a tool, we study 26 patients with follicular fluid 90 and 48 patients with 74 early embryos (2cell. 4cell,6cell,8cell, fusion)culture medium. Quantitative Detection of MMP9 secretion in follicular fluid and the embryo culture medium by ELISA.And then we applicate of statistical methods t test (for measurement data) orχ2 test (for count data) to analysis data, in order to observe early embryos different developmental stages ( 2cell,4cell,6cell,8cell,harmony embryos) MMP-9 secretion difference. Moreover, choosing follicular fluid and the embyro culture medium corresponding to the first grade 8cell embryo to transplant. Pregnancy accordance with the corresponding follicular fluid and the embryo medium MMP9 the 95% confidence interval for the critical value sector are high secretion and low secretion group,and to analysis of the two groups implantation rate and pregnancy rate and to further analyze the effect level of MMP-9 secretion to IVF-ET outcome.Results : (1) different developmental stage embryos corresponding follicular fluid and theembryo medium MMP9 secretion concentration : financial>8cell>6cell>4cell>2cell,and the comparison between groups was statistically significant (p <0.05); (2) Morphological assessment 8cell first grade embryos corresponding follicular fluid and embryo culture media concentration MMP-9 secretion is different,hypersecretion group implantation rate and pregnancy rate was significantly lower production group, the difference was statistically significant (p <0.05).Conclusion : follicular fluid and the fetus medium MMP9 the quantitative detection in clinical practice is expected to become an embryo selection parameters, to enhance embryo implantation rate.
