| Objection: Based on the analogy and biology mechanics characteristic of distal radioulnar joint and its violence with wrist and forearm in pronation, those kinds distal radioulnar joint dislocation or half dislocation (caused by the violence with forearm in pronation) are treated by the forearm in complete supination fixed with small splints. Discuss the advantages of this method and improve its curative effects.Method: According to the traditional curative effect evaluation among the treatment group, the forearm in neutrality position with small splints fixation group, the supinator position with plaster fixation group; meanwhile, according to the score evaluation of pain, the grip strength, the limitation degree of the forearm rotation, the distal radioulnar intervals and shortening distance of radius, the displacement degree of capitulum ulnae under the latter X-ray demonstration, carries on statistics processing.Result: In three groups of these cases, the worst curative effect is the neutral position fixation group. Compared the overall curative effect of the supinator position group with neutrality position with small splints fixation group, the former is better. In the forearm supination fixation groups, the curative effect of the small splints fixation group is superior to the plaster fixation one.Conclusion: While the forearm in complete supination, the radius shifts to the dorsal and distal side, and the capitulum ulnae is at the volar side of the notch ulnae. Meanwhile, the injuried ligaments in volar (the volar side of TFCC) and the Ligamentum capoulare caused by the violence in over pronation get relaxed and are apt to heal. In supination, the interosseous membrane relaxes and are apt to heal if injured in the distal radioulnar joint trauma cases. This therapeutic method fully takes advantages of the small splints fixation: Utilizes the bandage squeezing strength to the small splints in ulnar-radular and volar-dorsal directions, and the pressure of the rejoining bone pad and the styloid process of the ulnae cushion, to correct... |
